HomeMy WebLinkAboutCC RESO 9491RESOLUTION NO. 9491 RESOLUTION OF COMMENDATION WHEREAS, the City of National City was the only city in San Diego County ever to be represented with a City flag at the annual "Massing of the Colors" ceremony held in Balboa Park, and WHEREAS, Captain Dean Turner of the Police Reserves, Don Hove, Robert Strick and James Gorham served the City of National City with particular honor and distinction as color guards in the presentation of our City flag at this patriotic ceremony; NOW, THEREFORE, BE IT RESOLVED that the City Coun- cil of the City of National City hereby commends Captain Dean Turner, Don Hove, Robert Strick and James Gorham for repre- senting our community at the 13th Annual "Massing of the Colors" ceremony held in Balboa Park, November 5, 1967. PASSED AND ADOPTED this 14th day of November, 1967. ATTEST: AMENDMENT ATTACHED TO AND FORMING A PART OF GROUP POLICY NO. 3797, issued by CALIFORNIA-WESTERN STATES LIFE INSURANCE COMPANY, Sacramento, Cal- ifornia, to CITY OF NATIONAL CITY, National City, California. This Group Policy is hereby amended as follows: In the Schedule of Benefits (Page A) under the Employees' ,and Dependents' Major Medical Expense Benefit, the maximum allowable charge for hospi- tal room and board is increased from "$30.00 per day"to $40.00 per day." The above change shall be effective on October 1, 1967, provided that if an Employee is not then actively at work, such change shall not become effective with respect to such Employee and his dependents until the date the Employee returns to active work. Provided further that, if a dependent is confined in a hospital, nursing home, or similar institution on the date this change would have otherwise become effective, such change shall not become effective with respect to such dependent until the day immediately following termination of such confinement. Effective October 1, 1967, the SCHEDULE OF PREMIUMS is deleted and the at- tached SCHEDULE OF PREMIUMS is substituted therefor. In witness whereof, California -Western States Life Insurance Company has ex- ecuted this Amendment at Sacramento, California, on October 13, 1967.
