Subject: Change Order No. 14
Chance Order No. 14 provides for:
Engineering lighting assessment of the existing Kimball Park baseball field lighting conditions.
Payment for this change order will be as determined by force account.
Supporting Information:
During installation of the Kimball Park baseball field lighting the Subcontractor identified extensive rust and
corrosion to the existing ballast housing system and accompanying bracket structure at the top of the lighting
posts. The Contractor expressed that the corrosion may not allow for replacement of the ballast lighting without
causing additional damage to the housing system. To assess the potential of replacing the lighting system and
observe the extent of the corrosion, Josh Randall, S.E. from RTM performed a site visit to collect samples and verify
the integrity of the existing steel section.
Time Adiustment:
Zero (0) working days are to be added to the. contract.
Cost Adjustment:
The additional cost for this change in the scope of work has been agreed upon at a lump sum price of $1,260.00.
The cost for this change in the scope of work was determined by force account.
The pay method and amount have been reviewed by the Project Manager and are considered fair and reasonable. The
supporting documentation is on file with the project records.

This Change Order modifies Contractor's and the City's obligations and rights under the abovementioned
contract. This change order is not effective until approved by the City.
Time adjustment
Zero (0) working days are to be added to the contract.
Cost of Change Order:
The City and the Contractor agree that the Contractor proceeded under force account with assessing the
existing conditions of the Kimball Park baseball field lighting. The additional work includes but is not
limited to performing a site visit, taking coupons to verify integrity of the existing poles, and measuring
thickness of the steel to verify no loss of section. The total amount for all labor, material, equipment and
all other associated work is $1,260.00.

By signing this Change Order the Contractor confirms that he/she is completely familiar with and has fairly
negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the
equipment, materials and labor necessary to provide a complete work as described above at that the price
stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with
the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds,
insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the
original contract documents are incorporated herein. The signer for the Contractor confirms that he/she is
authorized to bind the Contractor to this Change Order.
