Payment for this change order was determined by agreed upon lump sum. Supportinq Information: The original plan maintenance access ramps were not feasible to construct with the existing 1.5:1 slope without having to build costly retaining walls.A new access ramp location was determined by HDR per RFI#68 that combined the two maintenance access ramps into one location without the need for retaining walls. Time Adjustment: Two(2)working days are to be added to the contract. Cost Adjustment: The additional cost for this change in the scope of work has been agreed upon at a lump sum price of$24,150.00. The pay method and amount have been reviewed by the Project Manager and are considered fair and reasonable.The supporting documentation is on file with the project records. This change order is not effective until approved by the City. Time adjustment: Two(2)working days are be added to the contract. Cost of Change Order: The City and the Contractor agree that the Contractor shall proceed with concrete maintenance access ramp per attached RFI#68 for an approximate 345 square feet.The total amount for all labor, material, equipment and all other associated work is$24,150.00. Recommended by Project Manager: osh Johnson, PPC Date: 3/11/25 By signing this Change Order the Contra or confirms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein. Section: RFI#68-Details for Access Roads. Wright Construction is requesting clarification pertaining to the access roads that are shown on the project plans.On page 7 of the project plans it is showing an 8 foot wide access road is to be installed on the left and right side of the wingwalls on the downstream section and provides a detail for the access road on sheet 3 of the plans.Wright is unable to see how the access road is to be constructed as shown on the plans as the areas that are shown on the plans are a 1:1 to 1:1.5 slope and the details that are shown on sheet 3 are showing a cross slope ofa 2 percent fall of which if the contractor was to build the access road as shown on the plans,there is the possibility of the access road being undermined by the flow within the creek as there is nothing containing the aggregate that is to be utilized for building of the access road.Wright is also requesting additional details for the articulated concrete block that is to be installed at the bottom of the slope for the access road as shown on sheet 7 of the plans. If the City can please provide clear direction on the details for the access road as Wright feels that the access roads cannot be built within the areas shown on the plans due to the existing conditions for the areas that the access road is to be constructed?When you are able to,please provide the requested information. The ramp is to be 11.5' wide and 8" thick with 560-C-3250 concrete mix with a metal tine rake finish. Base bid access road #1 and #2 to be removed from plans. Please provide pricing for the access road concrete ramp detail. This RFI is not an authorization to proceed with changed work.Separate written authorization must be received from the City to proceed with any additional work. No extra work will commence prior to receipt of written authorization from the City.
