Plaza Blvd intersection at approximate station 3+25. Payment for this change order was determined by agreed upon lump sum. Supporting Information: Sweetwater Authority provided disconnect and reconnect of a water line to allow the precast box culvert to be installed through the intersection of Paradise Valley Road and E. Plaza Blvd.With the project nearing completion Sweetwater Authority could not obtain crews fast enough to repair the trench without holding up the project and have opted for National City to provide trench repair under the Contract and then reimburse National City.See attached email with confirmation. Time Adjustment: Two(2)working days are to be added to the contract. Cost Adjustment: The additional cost for this change in the scope of work has been agreed upon at a lump sum price of$13,584.88. The pay method and amount have been reviewed by the Project Manager and are considered fair and reasonable.The supporting documentation is on file with the project records. Cost of Change Order: The City and the Contractor agree that the Contractor shall proceed with repairing Sweetwater Authority trench within Paradise Valley Road and E. Plaza Blvd intersection at approximate station 3+25. Work to included but not limited to removal of temporary asphalt, installation of base course asphalt and 2"grind and overlay for trench limits.The total amount for all labor, material, equipment and all other associated work is$13,584.88. Recommended by Project Manager; - Os Johnson, PPC Date: 4/1/25 By signing this Change Order the Contra r confirms that he/she is completely familiar with and has fairly negotiated its price,terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
