Subject: Change Order No.35
Change Order No.35 provides for:
Proceed with installation of 4 traffic loop detectors for Westbound Paradise Valley Road at the intersection of
Paradise Valley Rd and E.Plaza Blvd.
Payment for this change order was agreed upon at a lump sum price.
Supporting Information:
The plan revisions provided by HDR on February 14,2024,relocated the precast box culvert approximately 4'
North of the originally planned location to avoid conflicts with the existing VCP sewer main.The realignment
moved the trench zone to conflict with the existing traffic loop detectors and the homerun to the traffic signal
system for Westbound traffic on Paradise Valley Road.
Time Adjustment:
One(1)working days is to be added to the contract.
Cost Adjustment:
The additional cost for this change in the scope of work has been agreed upon at a lump sum price of$3,150.00.
The pay method and amount have been reviewed by the Project Manager and are considered fair and reasonable.The
supporting documentation is on file with the project records.

This Change Order modifies Contractor's and the City's obligations and rights under the abovementioned
contract. This change order is not effective until approved by the City.
Time adjustment:
One(1)working days is to be added to the contract.
Cost of Change Order:
The City and the Contractor agree that the Contractor shall proceed with installation of 4 traffic loop
detectors for Westbound Paradise Valley Road at the intersection of Paradise Valley Rd and E. Plaza
Blvd.The additional work includes but is not limited to placing traffic control devices,sawcutting and
installing traffic loop detectors and connecting the loop detectors to the existing traffic signal system.
The total amount for all labor, material,equipment and all other associated work is$3,150.00.

By signing this Change Order the Contrac or confirms that he/she is completely familiar with and has fairly
negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the
equipment, materials and labor necessary to provide a complete work as described above at that the price
stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with
the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds,
insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the
original contract documents are incorporated herein. The signer for the Contractor confirms that he/she is
authorized to bind the Contractor to this Change Order.
