Subject: Change Order No.1
Change Order No. I provides for:
Proceeding with horizontal directional drilling approximately 50 linear feet of new 4"Schedule 80 PVC conduit
from last pullbox, beneath plaza improvements and into electrical room.
Payment for this change order will be as agreed upon lump sum.
Supporting Information:
During the installation of conduit and pull boxes,the Contractor was unable to locate the 4" PVC conduit as shown
on the construction plans,which was intended to run from the edge of the sidewalk beneath the park plaza's
concrete improvements and into the electrical building.
To investigate further,the Contractor potholed along the entire front edge of the concrete plaza.This investigation
revealed a direct-burial electrical wire in the approximate location where the conduit was expected,indicating a
deviation from the design plans.
To install the new conduit to the electrical building,two methods were evaluated:directional drilling and open
trenching(which would require removal and replacement of decorative concrete).After a thorough review of both
options,including cost and schedule impacts,directional drilling was determined to be the most cost-effective and
time-efficient solution.
Time Adjustment:
Five(5)working days are to be added to the contract.
Cost Adjustment:
The additional cost for this change in the scope of work has been agreed upon at a lump sum price of$17,813.53.
The pay method and amount have been reviewed by the Project Manager and are considered fair and reasonable.The
supporting documentation is on file with the project records.

This Change Order modifies Contractor's and the City's obligations and rights under the abovementioned
contract. This change order is not effective until approved by the City.
Time adjustment:
Five(5)working days are to be added to the contract.
Cost of Change Order:
The City and the Contractor agree that the Contractor shall proceed with horizontal directional drilling
approximately S0 linear feet of new 4"Schedule 80 PVC conduit from last pullbox, beneath plaza
improvements and into electrical room.The total amount for all labor, material, equipment and all other
associated work is$17,813.53.

By signing this Change Order the Contrac or confirms that he/she is completely familiar with and has fairly
negotiated its price,terms and conditions. If this Change Order is approved, Contractor will provide all of the
equipment, materials and labor necessary to provide a complete work as described above at that the price
stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with
the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds,
insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the
original contract documents are incorporated herein. The signer for the Contractor confirms that he/she is
authorized to bind the Contractor to this Change Order.

Upon extensive excavation in the area behind home plate,we have not found any existing 4"conduit as
described and as shown on the project drawings,sheet 2,Note 3.It appears that the cables are directly buried under
the dirt and concrete.
To keep the concrete in tact and minimize disturbance,We recommend approval of a change order to hire
a Horizontal Directional Driller to install a new 4"SCH 80 PVC Conduit from the location of intercept to the
MUSCO Panel.A DIR registered HDD contractor,familiar with this project has supplied an estimate which is
included with this RFI.

PROJECT SCOPE OF WORK:
• Utility Delineation:
o USA Dig Alert:Devco will secure and maintain a valid USA dig alert ticket to attempt to capture all underground utilities that are on the
property(For public utilities only).
o Utility Delineation Private Locator:
Devco will supply a private locator for the private property to attempt to capture all existing utilities that may intersect the directional
bore path.
• Utility Verification via Potholing;
o Potholing Operations:Devco will conduct potholing via 8"core method to confirm the locations of the existing utilities that intersect or
run parallel to the directional bore paths,marked by private locator and USA dig alert.
o Backfilling:After potholing,the potholes will be backfilled with utility sand or a 50/50 mix,compacted,and the 8"cores will be
restored and sealed with concrete sealant as final restoration by Devco.
o Devco will haul and dispose of all pothole mud.
• Excavation and Preparation:
o Bore Pits:Devco will provide labor and equipment necessary to excavate all directional bore pits required for the directional boring
operations.
o Spoils Management:All spoils from the directional bore pits will be stockpiled on the jobsite and used for backfilling.
• Directional Boring:
o Boring Distance:Devco will undertake directional boring at(1)location for approximately 46 LF.
o Devco will supply and assemble roughly 46'of(1)4"BoreGard Schedule 40 PVC conduit for the directional boring operations only.
o Devco will haul and dispose of all directional boring fluid.
• Restoration:
o All bore pits will be backfilled and compacted with native soil,and restored to trench width only.
o All bore pits will be restored to trench width only,and repaved by Devco.
