Payment for this change order was determined by bid unit price and measured LF. Supporting Information: During construction additional areas of damaged joint sealant adjacent to the work area were discovered and added to the scope.See attached exhibit for approximate locations. Time Adjustment: Zero(0)working days are to be added to the contract. Cost Adjustment: The additional cost for this change in the scope of work has been agreed upon at unit bid price of$18 LF for an additional 93 LF for a total of$1,674.00 The pay method and amount have been reviewed by the Project Manager and are considered fair and reasonable.The supporting documentation is on file with the project records. This change order is not effective until approved by the City. Time adjustment: Zero(0)working days are be added to the contract. Cost of Change Order: The City and the Contractor agree that the Contractor shall proceed with additional removal and replacement of joint sealant in areas adjacent to bid limits for an approximate 93 additional linear feet. The total amount for all labor, material,equipment and all other associated work is$1,674.00. Recommended by Project Manager.?/ Josh Johnson, PPC Date: 3/14/25 By signing this Change Order the Contractor confirms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein. Area depicted in Exhibit A (see attached). Reason for Change: Requested by the Police Department. Entire walkway surrounding the building was resealed except for two smaller areas at each gate entrance. Proposed Change: Removed and replaced an additional 93 linear feet of joint sealant located at gated parking area entrances (see attached).
