This Change Order modifies the Contractor's and the City's obligations and rights under the above-mentioned contract. This change order is not effective until approved by the City. Payment for this change order will be at a lump sum price identified. Supporting Information: This extra work was required due to a crack in the existing sewer VCP. Time Adjustment: Two(2)contract days were added to the contract time to coordinate the work. Added By David Goan on Oct 22,2024 Change Order#9-P-2 Upsizing 8"to 10"-DMI set traffic control per SDRSD TCP-15 with added bike lane closed,share the road,and steel plate ahead signs.Contractor set bypass pumps in center lane and continued sewer by pass from manhole 1953 to 1054.R&C Structures Wyatt Nevitt on site to pour manhole base at 1954 per SDRSD SM-03 and approved submittal.Superior Ready mix on site at approximately 9:30am with 4000psi concrete 57P.Manhole base was poured and finished by approximately 11:00am.DMI continued bypass until 3:00pm at which time sewer bypass was removed and sewage ran through poured manhole base#1954.DMI was notified if any damaged occurred to channel or trough of manhole base due to releasing bypass before concrete at fully cured,DMI would be responsible to replace manhole base.DMI moved excess spoiled dirt material along new sewer trench to stockpile location near spigot area.Contractor worked on removing excess spoils from new curb and gutter improvements along multi-use trail.City Crews were on site to CCTV existing 15"clay pipe down stream of manhole 1054.It was determined cracks on clay pipe terminated approximately 3'from bell end and approximately 5.5'from manhole.DMI was directed to proceed on disputed work to remove cracked 15"clay pipe and replace with 15"PVC with fitted wye to proved access for bypass to pour new manhole base at manhole#1054.Disputed Time and Materials work will start on 10/22/2024. ICCS contacted SWA Adam Johns and notified SWA excavation would be taking place around 12"AC line.Adam visited the site and said he would return on 10/22/2024. Added By David Goan on Oct 29,2024 Plaza-Perry Electric poured three decretive column light foundations with Robertsons 3250 PSI concrete.Contractor hung cages and set anchor bolts per project plans and approved submittal.Rebar cages and anchor bolts were inspected by ICCS with no exceptions noted.See attached photos and submittal.Perry had augured decretive column light foundations on a previous date. Added By David Goan on Oct 29,2024 Plaza-DMI requested ICCS to inspect forms for decretive concrete banding so Perry Electric could pour decretive Column light foundations.ICCS inspected forms and found forms not to be set per project plans.ICCS notified DMI that forms were not set correctly and to have DMI crews adjust forms to meet dimensions on project plans before pouring foundations.DMI adjusted forms.ICCS re-checked forms and it appeared adjustments had been made.ICCS informed DMI and Perry that ultimately it is the contractors responsibility to ensure the foundations and forms are set per project plans.Any errors or non compliant work is not the responsibility of ICCS.DMI directed Perry Electric to pour the decretive column light foundations. Thursday Added By David Goan on Nov 1,2024 Plaza-DMI re-compacted slope behind maintenance access road between stations 23+10 and 24+48 with back hoe and sheep's foot compaction wheel.Slope had been previously compacted before maintenance road was installed however,DMI has placed loose material on slope and ICCS had requested DMI to re-compact.Loose material on slope behind Thickened edge sidewalk still needs to be compacted at later date. The additional tests verified that mastic installed on manhole 1954 did not seal correctly and mastic below one shaft on manhole 1054 did not set property.The contractor added mastic to manhole 1954 and reset shaft with new mastic on manhole 1054.ICCS on site to observe 2nd Negative pressure tests at both manholes.A vacuum of 10" was drawn in both manholes each manhole did not drop below 9"within 75 seconds.Both Manholes passed Negative air pressure per project specifications 306-7.9.No exceptions noted.Mar-con/R&C Structures Manpower Jesus Gutierrez-4.5 Firs,Wyatt Neurtt-2 Hrs.DMI installed mortar on all joints and lifting holes on the outside of the manholes.Mortar will be installed on the insides of the manholes at later date.DMI placed 3/4"rock bedding on and around sewer pipes outside of manholes,ready to backfill.DMI used sheep's foot compaction wheel,compacting remainder of sewer trench and surrounding area on western slope to prepare for final finish grade prior to weed kill and bark mulch installation.Contractor removed Bypass system,traffic control and started disassembling bypass highline.Contractor cautioned off work area. Added By David Goan on Nov 1,2024 North Side 4th street between Modular wetlands and A-4 cleanout.-DMI installed mortar at angle point in 24"RCP at approximate station 15+36 per project plans sheet 56.DMI provided a camera for inspection.ICCS inspected mortar at angle point.No exceptions noted.However,Concrete lug at modular wetlands at tie in point at approximate station 15+18 needs significant work.While pouring the lug the sano tube failed.ICCS informed DMI that the tie in is not acceptable and additional work will be needed before excepted. Added By David Goan on Nov 1,2024 Change Order#9-P-2 Upsizing 8"to 10"-DMI continued to use sheep's foot compaction wheel on slope to compact sewer trench and surrounding area.Contractor dressed up slope to prepare for weed kill and bark mulch.DMI backfilled around manhole 1954 water conditioning existing dirt material,compacting in lifts with walk behind whacker and sheep's foot compaction wheel.DMI's underground crew performed Air pressure test on new 10"SDR35 sewer pipe between manholes 1954 and 1054 and passed inspections per green book 306-7.8.2.4.No exceptions noted.DMI removed shoring from excavation around manhole 1054.Contractor cautioned of excavation with 6'temporary chain link fence and will begin backfill on 11/04/2024. NOTE:The contractor will be allowed 15 days from the date of this report in which to protest in writing the correctness of this statement,otherwise it shall be deemed to have been accepted as correct.
