Camacho Recreation Center, On August 25, 2000 a final inspection was completed and the work was found to be in accordance with the approved plans and specifications. Change Order No.1 was written to adjust the Additive Alternate Item 1, Deck Repair and Replacement, bid quantity to the actual quantity of work performed, and to add Line Item No. 2, Install Roof Drain. See attached Change Order for a more detailed description. Change Order No.1, effectively increases the contract amount by $144.00 from the original amount of $49,277.00 to a new contract amount of '$49,421.00. After demolition of the previous roofing system it was determined that only three sheets of plywood decking (96 Square Feet) warranted replacement. Line Item No. 2 has been added to the contract to provide additional drainage to two areas of the roof that are currently ponding water. This original roof design problem is being addressed at this time to avoid deterioration of the new roofing material in these areas. CONTRACT ADJUSTMENT: As a result of this change order the contract price is adjusted as follows: 1. The contract price is increased in the amount of $144.00 from the current contract price of $49,277.00 to a new contract price of $49,421.00. 2. The contract time is increased by 48 working days due to the addition of the Roof Drain line item to the contract. The contract completion date was previously established as Monday, June 19, 2000 and has been extended to Friday, August 25, 2000. This change order shall be considered full compensation for furnishing and installing the materials, labor, tools and equipment, profit, overhead, and all incidentals for performing the work described above. Roejack Roofing, Inc. will not be entitled to damages or additional payment for delays as described in the 1997 edition of Standard Specifications for Public Works Construction, Section 6-6.3, for performing the work as described above. This Supplemental Questionnaire will be used to evaluate whether the event or sponsoring organization for a TUP meets the criteria for a fee waiver.
