On December 5, 1995 the City Council adopted Staffs recommendation to
extend the Ambulance Services Agreement with Hartson Medical Services
and directed Staff to negotiate a three (3) year extension with a six
(6) month cancellation clause.
Staff has reached an agreement with Hartson Medical Services to extend
the current contract as stipulated in the attached amendment to
agreement document.

CITY OF NATIONAL CITY AMBULANCE SERVICES CONTRACT
STATUS REPORT
NOVEMBER 15, 1995
BACKGROUND:
The City's ambulance services contract with Hartson Medical
Services will expire on October 1, 1996. To ensure the continued
delivery of these services, the City must either begin the request
for proposal process or enter into negotiations with Hartson to
extend the current contract.
Hartson, at no cost to the City, has been providing basic and
advanced (paramedic) life support ambulance services to the City
since October 1, 1983.
The health care industry is currently undergoing major changes in
the way that medical services are delivered. These changes are
impacting all facets of the medical care delivery system including
"Pre -Hospital Care", which directly involves the City through the
Fire Department's First Responder Program and indirectly through
the City's ambulance subcontractor, Hartson Medical Services.
In order to ensure that quality ambulance services continue to be
provided as economically and efficiently as possible, a
reassessment of the City's pre -hospital medical care delivery
system must be completed taking into consideration the various
issues that are currently impacting pre -hospital health care. Some
of these issues are:
- Establishment of health service areas
- Compliance with State and local regulations
Coordination with health care providers
Coordination with third party payors
Public safety provider reimbursements
- Coordination with private ambulance services
Potential for private/public partnerships

Several Cities and Special Districts in the County will soon be
addressing this problem of how best to deliver these services in
the current health care environment. An advantage can be gained by
studying the approaches that will be taken by these Emergency
Medical Service Areas (EMSA's) in the next 24 - 36 month period.

REGIONAL SYSTEM DESIGNS:
The City of San Diego is one of the Emergency Medical Service Areas
that will soon begin to respond to the issue of pre -hospital care
in a managed care environment. The San Diego EMSA is not only the
largest area in the County, but as our neighbor, also has the
greatest potential to influence the design of the National City
system. Aside from the obvious difference in size between the two
service areas, there are basic service requirements that are
similar. The City of San Diego is planning to study, design,
competitively bid and implement a new pre -hospital medical care
delivery system by July 1, 1997. To accomplish this task they plan
to hire a consultant at a fee of approximately $100,000 to conduct
a comprehensive study of the City's pre -hospital medical care
delivery system needs considering many of the issues described
above. As a result of this study, a recommendation on system
design will be presented followed by a request for proposal process
and award of a service contract. The delivery system that will
evolve from this process is expected to more clearly address, from
a regional perspective, the impact that managed care will have on
pre -hospital medical care system design.
STAFF RECOMMENDATION:
Negotiate with Hartson Medical Services and obtain a three (3) year
extension to the current ambulance services contract beginning
October 1, 1996.
JUSTIFICATION:
This extension will allow staff to study not only the City of San
Diego's new delivery system, but others as well. Reviewing the
results of the San Diego system will ensure that National City's
system operates not only efficiently, but in concert with
neighboring systems, while obtaining a clearer assessment of the
impacts that managed care will have on the delivery of pre -hospital
care services.
The San Diego County Emergency Medical Services Authority has been
consulted and concurs with the observations and conclusions
described in this report.

AUTHORIZING THE MAYOR TO EXECUTE AN
AMENDMENT TO THE AGREEMENT BETWEEN THE
CITY OF NATIONAL CITY AND
HARTSON MEDICAL SERVICES FOR BASIC
AND ADVANCED LIFE SUPPORT SERVICES
WHEREAS, on October 1, 1987, the City and Med-Trans, dba Hartson
Medical Services ("Hartson") entered into an agreement entitled "Agreement between the
City of National City and Hartson Medical Services for Basic and Advanced Life Support
Services"; and
WHEREAS, the term of said Agreement has subsequently been extended
from time to time, with the current term of said Agreement to expire on September 30,
1996; and
WHEREAS, the City and Hartson now wish to amend Section 17 of said
Agreement, concerning the extension of the term of said Agreement, to permit the City
to extend the Agreement for an additional three years, subject to either party having a
right of cancellation upon 180 days' prior written notice; and
WHEREAS, the City and Hartson wish to further amend said Agreement,
to provide that Hartson shall perform various additional duties not addressed in the
current Agreement.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City
of National City that the Mayor is hereby authorized to execute an Amendment to
Agreement between the City of National City and Hartson Medical Services for Basic and
Advanced Life Support Services. Said Amendment to Agreement is on file in the Office
of the City Clerk.

AMENDMENT TO AGREEMENT
BETWEEN THE
CITY OF NATIONAL CITY
AND
HARTSON MEDICAL SERVICES
FOR BASIC AND ADVANCED LIFE SUPPORT SERVICES
This Amendment to Agreement is made and entered into this 21st day
of May , 1996, by and between the City of National
City, a municipal corporation, hereinafter referred to as "CITY" and Med-Trans, a
California limited partnership, dba Hartson Medical Services, hereinafter referred to as
"HARTSON".
RECITALS
A. On October 1, 1987, CITY and HARTSON entered into an
agreement entitled "Agreement between the City of National City and Hartson Medical
Services for Basic and Advanced Life Support Services."
B. The term of said Agreement has subsequently been extended from
time to time, with the current term of said Agreement to expire on September 30,
1996.
C. CITY and HARTSON now wish to amend Section 17 of said
Agreement, concerning the extension of the term of said Agreement.
NOW, THEREFORE, in consideration of the mutual benefit to be derived
therefrom, CITY and HARTSON agree to amend Section 17 of the Agreement
between the City of National City and Hartson Medical Services for Basic and
Advanced Life Support Services, to read as follows:
17. EXTENSION: CITY shall be entitled upon giving ninety (90)
days' prior written notice to HARTSON, to extend the term of this Agreement
for a period of three additional years beyond the current expiration date of
September 30, '1996, provided that either party may, at any time during the
extended term, cancel this extension by giving one hundred eighty (180) days'
prior written notice of such cancellation. In the event notice of cancellation
given by CITY as provided for herein, HARTSON shall continue to provide
service under the terms of this Agreement for the remaining term of the
extension.
D. In addition, HARTSON agrees to the following:
1
1. To immediately place, in the advanced life support Unit
dedicated to the National City Service Area, radio equipment capable of
communicating on the CITY'S 800 MHz Radio System.
2. To re -supply or reimburse the CITY'S Fire Department for
consumable/disposable medical supplies used when Fire Department and Hartson
Units are both on scene, including disposable defibrillator supplies.
3. To dispose of biological/medical waste materials generated
by the CITY'S Fire Department on EMS related calls.
4. To design, carry out and report on a quarterly basis the
results of a comprehensive public education program designed to meet the specific
needs of National City citizens. This Program to be measured using the attached
Exhibit "A" "Performance Indicator Format", reviewed and approved by the CITY'S
Fire Chief during the month of March each year.
5. To continue to provide "First Responder" billing services.
6. To adjust current "First Responder Fee" in accordance with
Section three (3) of the current agreement with final "First Responder Fee" adjustment
to equal not less than $32.00. Actual charges collected and subsequently paid to
National City will be based on the thirteenth month's percentages for National City
accounts, though not less than forty percent (40%) ($12.80 per call).
7. HARTSON agrees to bill "First Responder Fee" for all
ambulance transports and non-transports/assessment calls when first responder
services were provided and a HARTSON Unit responded.
8. Should the CITY desire, HARTSON agrees to explore using
a separate invoice when billing for first responder services with payments mailed
directly to the CITY.
9. HARTSON agrees to provide spine stabilization boards (back
boards) to the CITY. CITY will label each board "Return to National City Fire
Department" and provide storage space at each station for additional back boards.
E. With the foregoing exceptions, each and every provision of the
"Agreement between the City of National City and Hartson Medical Services for Basic
and Advanced Life Support Services", dated October 1, 1987, shall remain in full
force and effect.
