Notice of Completion 09R A-200 (9/99) Resolution No. 2003-111 Explanation: August 6, 2002 by Resolution No. 2002-111, the City Council awarded a contract in the amount of $229,300.00 to ASSI Security, Inc. For the Civic Center and Police Facility Security and Fire System Enhancements, Specification No. 02-6. On May 30, 2003 a final inspection was completed and the work was found to be in accordance with the approved plans and specifications. Change Order No. 1 was written to include Line Item No. 8 to provide compensation to the contractor for furnishing and installing three complete ADA Facility Entrance System similar to the ADA entrance system on the second floor of the Civic Center from National City Boulevard pedestrian bridge. Change Order No. 2 was written to add Line Item No. 9 by $8,730.00 at the City's request to provide compensation to the contractor for furnishing and installing one additional proximity card reader and associated door hardware, additional prewiring for the proposed personnel remodel area on the garden level, 8 panic buttons and associated wiring, and to supply 150 additional proximity security access cards. Change Order No. 3 was written to add Line Item No. 10 by $1,815.00 at the City's request to provide compensation to the contractor for furnishing and installing one additional proximity card reader and associated door hardware, for the lobby area. Change Order No. 4 was written to add Line No. 11 by $3,755.00 at the City's request to provide compensation to the Contractor for relocating the fire alarm panels. Change Order No. 5 was written to add Line Item No. 12 by $3,820.00 at the City's request to provide compensation for the addition of two outdoor color cameras, two additional keypads, and one additional request to exit device. RESOLUTION NO. 2003 —111 RESOLUTION OF THE CITY COUNCIL OF THE CITY OF NATIONAL CITY AUTHORIZING THE FILING OF A NOTICE OF COMPLETION FOR THE CIVIC CENTER AND POLICE FACILITY SECURITY AND FIRE SYSTEM ENHANCEMENTS (Engineering Spec. No. 02-06) BE IT RESOLVED by the City Council of the City of National City, California, as follows: It appearing to the satisfaction of the Engineering Department that all work required to be done by ASSI SECURITY, INC., Engineering Department Specification No. 02-06, for the Civic Center and Police Facility Security and Fire System Enhancements has been completed, the City Council of National City hereby accepts said work and authorizes the filing of a Notice of Completion and orders that payment for said work be made in accordance with said contract.
