EXPLANATION
Fire
The City's Ambulance Services Agreement with American Medical Response (AMR) will expire on
November 30, 2004. AMR, at no cost to the City, has been providing basic and advanced life support
ambulance transportation to the City since October 1, 1987. As a continuous measure and to ensure that
ambulance services to the City are not interrupted, Staff is requesting permission to invoke Section 17
entitled "Extension" of the agreement. This Section allows the City to unilaterally extend the Contract
twice for one (1) year each period if needed. Each one (1) year extension requires the mutual consent of
both parties to extend the Contract Term. Written notice of concurrent agreement to enact Section 17 for
the one (1) year period beginning December 1, 2004 has been received from AMR.

RESOLUTION OF THE CITY COUNCIL
OF THE CITY OF NATIONAL CITY
AUTHORIZING THE MAYOR TO EXECUTE AN
EXTENSION OF THE CITY'S AMBULANCE SERVICES
AGREEMENT WITH AMERICAN MEDICAL RESPONSE FOR
BASIC AND ADVANCED LIFE SUPPORT SERVICES
WHEREAS, on October 1, 1987, the City and Hartson Medical Services
("Hartson") entered into an agreement entitled "Agreement Between the City of National
City and Hartson Medical Services for Basic and Advanced Life Support Services".
Said Agreement has been assigned to Med-Trans, dba American Medical Response
("AMR"); and
WHEREAS, the term of said Agreement was subsequently extended from
time to time, with the current term of said Agreement to expire on November 30, 2004;
and
WHEREAS, the City and AMR now wish to extend the Agreement for an
additional one-year period, pursuant to Section 17 of the Agreement.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of
National City that the Mayor is hereby authorized to execute a written notice extending
the Agreement between the City of National City and AMR for Basic and Advanced Life
Support Services, up to and including November 30, 2005. Said Amendment is on file
in the Office of the City Clerk.

AMENDMENT TO AGREEMENT
BETWEEN THE CITY OF NATIONAL CITY AND
LAIDLAW MEDICAL TRANSPORTATION, INC.,
DBA AMERICAN MEDICAL RESPONSE (AMR),
FORMERLY HARTSON MEDICAL SERVICES (MED-TRANS), FOR
BASIC AND ADVANCED LIFE SUPPORT AMBULANCE SERVICE
DRISINAL,
This Amendment to Agreement is entered into this 19TH day of
OCTOBER 2004, by and between the City of National City ("CITY")
and Laidlaw Medical Tansportation, Inc., a California Corporation, dba American
Medical Response ("AMR").
RECITALS
A. WHEREAS, on October 1, 1987, CITY and HARTSON MEDICAL
SERVICES entered into an agreement entitled "Agreement between the
City of National City and Hartson Medical Services for Basic and Advanced
Life Support Services"; and
B. WHEREAS, said agreement was assigned to Med-Trans, dba American
Medical Response ("AMR"); and
C. WHEREAS, said agreement is currently assigned to Laidlaw Medical
Tansportation, Inc., dba American Medical Response ("AMR"); and
D. WHEREAS, the term of said Agreement has been subsequently extended
from time to time with the current term of said Agreement to expire on
November 30, 2004; and
E. WHEREAS, the CITY and AMR now wish to amend Section 17 of said
Agreement, concerning the extension of the term of said Agreement.
NOW, THEREFORE, in consideration of the mutual benefit to be
derived therefrom, CITY and AMR agree to amend Section 17 of the "Agreement
between the City of National City and AMR for Basic and Advanced Life Support
Services," to include the following language:
EXTENSION: The term of this Agreement shall be extended
for a period of one (1) year beyond the current expiration date
to November 30, 2005.
Except as otherwise provided herein, all of the provisions of the
Agreement for Basic and Advanced Life Support Services between the City of
National City and Hartson Medical Services, dated October 1, 1987, and amended
Basic & Advanced Life 1 Third Amendment to Agreement
Support Ambulance Service American Medical Response
on May 21, 1996, March 21, 2000, November 14, 2000, and November 30, 2004,
and which has been assigned to Laidlaw Medical Tansportation, Inc., dba
American Medical Response, shall remain in full force and effect.
IN WITNESS WHEREOF, the parties hereto have caused this
Amendment to be executed the day and year first herein set forth.

contract by and between the City of National City and American Medical Response
(AMR), we concurrently agree to exercise the option to extend the contract for one (1)
year beginning December 1, 2004.

Dear Mr. Murphy,
On October 19, 2004, Resolution No. 2004-197 was passed and adopted
by the City Council of the City of National City, authorizing the Mayor to
execute an extension of the City's ambulance services agreement with
American Medical Response for basic and advanced life support services.
We are forwarding a certified copy of the above Resolution and one fully
executed Agreement.
