1. Concrete Vapor Sealer due to high level of moisture in the slab on grade 1 COR 091.001 ]$ 8,392
REASON FOR CHANGE:
1. After conducting calcium chloride tests to determine the moisture level in the slab on grade, it
was determined that the levels were higher than what was acceptable by the flooring
manufactures. The need for concrete vapor sealers for the carpet, vinyl tile and the rubber
flooring were needed in order to bring the moisture level to the manufactures acceptable level.
COST RECONCILIATION:
Costs were reviewed by the architect and CM and negotiated to the satisfaction of the City.

• The cost for JJJ Flooring's scope of work in the amount of $7,913.00 is approved plus M+H's
mark-up of 5% for Overhead & Profit and 1% for Bond Premium for a total of $8,392.00.

The Contract adjustment required as a result of: Concrete Vapor Sealer
Attached Product Data
Ten calcium chloride test have been performed to determine the concrete vapor emission levels.
The test results indicated that the moisture content at the carpet, vinyl tile and rubber tile were
well above the manufacture allowed moisture emission levels.
The cost of this change order is associated with the additional labor and material required to
furnish and install a concrete vapor sealer at carpet, vinyl tile and rubber tile areas.
Change Order Request No. 091.001 is $8,392
The incorporation of this revision into the contractual scope of work will require 0 working day not
included in our current schedule. The job schedule will be adjusted accordingly to show the effect
of this revision on the final project completion date. The terms (cost and schedule impact) of this
change order request are subject to review and a re -quote if not accepted within 14 days of its
issuance.
We HAVE NOT proceeded with this revised work. Please issue an authorization and/or
change order.

LEES EVERSEAL
Lees Universal Concrete and Floor Sealer is a latex -based sealer designed for ensuring the
adhesive bond for installations where potential problems exist. EverSeal has excellent resistance
to moisture penetration, adhesive bleed -through from latex -based adhesives, and dust and oil
contamination. It is ideal for use in sensitive environments; it has very low odor and low VOCs
(volatile organic compounds) and contains and antimicrobial agent. EverSeal is designed for use
with all Lees' installation products: Lees Wet Set Adhesive. Lees Squared Pressure Sensitive
Adhesive, Lees Thermntapc, and Leas NeoFloor Adhesive. EverSeal is packaged in four -gallon
containers; spread rate is approximately 40 yards per gallon per coat.
One Coat of EverSeal is Recommended for the Following Conditions:
Dusty Sabfioors - EverSeal .seals out dusty. dirty subfloors. pmviding a clean substrate cm which
to install any of the Lees adhesives and carpet products. EverSeal will also seal Cryperete patches
and oil contamination over wood subfloors.
Porous Suhftoors - EverSeal will seal porous subfloors, preventing the loss of adhesive into the
subfloor. Porous subfloors can include unsealed concrete or hardwood, or plywood subfloors.
EverSeal will also seal concrete patching compounds.
Latex -based Adhesives - Lees EverSeal is guaranteed to seal any existing latex. adhesive residue
that may remain on the subfloor. EverSeal serves as a barrier between the old and new adhesives
to prevent bleed -through or reactions with the new adhesive. EverSeal will also deaden residual
adhesive tack to provide easier installation of the new carpet. NOTE: Lees EverSeal is not
warranted for use aver cut -back or black solvent -based adhesives. To seal old cut•buck or black
solvent-basrd adhesives, use Zees SureSeal subfloor sealer.
Two Coats of EverSeal Are Required for the Following Conditions:
Elevated pH (Alkalinity) Levels—.EverScal will provide a harrier coat to prevent alkaline salts
in the slab from reacting with the adhesive_ If the pH level of the subfloor is between 9 and 14,
mop the floor with a vinegar and water solution (eight ounces of vinegar to one gallon of water) to
remove alkalinity on the surface of the slab. Allow this solution to dry completely, then use two
coats of Lees EverSeal to seal the surface of the concrete (see EverSeal installation instructions).

Elevated Moisture Levels - EverSeal is recommended for any subfloor with a calcium chloride
test result up to 8 pounds of moisture per 1.,000 square feet per 24 hours when used with Lees Wet
Set Adhesive. installations using pressure sensitive adhesives, tape systems, or any of Lees' other
adhesive systems cal be protected up to 8 pounds of moisture when using lees EverSeal.
However- tees Technical Service must be consulted if the moisture reading is above 5 pounds for
these types of installations_ Lees will assist the contractor in determining the best application
method based on site conditions. If two coats of EverSeal are applied, with both coats receiving
24 hours of curing time, EverSeal is guaranteed for the lifetime of the carpet as a moisture barrier
between the subfloor and Lees adhesives. Please note that the calcium chloride test measures
moisture, not hydrostatic pressure. EverSeal is not guaranteed against hydmstatic pressure.
Warranty
When used according to instructions and in conjunction with Lees' carpets and adhesives, the
Lees Lifetime Warranty extends to the adhesive bond of the carpet to the subfloor. This warranty
covets EvcrS al's ability seal out moisture emission up to 8 pounds per I,000.square feet per 24
hours as measured by a calcium chloride test (ASTM F 1869-98) when used with Lees Adhesives.
(For adhesives other than Lees Wet Set, contact Lees Technical Service if the moisture reading is
between 5 and 8 to determine the hest application method.) If the moisture level is over the
rcommendcd limits, other means trust be employed to curtail .moisture emissions before
installation. NOTE: Lees Technical Service must be consulted if the moisture reading Is
above 5 pounds for the types of installations. Lees will assist the contractor in determining
the best application method based on sight conditions.
EverSeal is also warranted to also stop alkaline salts from rising to the .surface of the slab,
providing the slab is cleaned with the vinegar and water solution before application. Also,
EverSeal is guaranteed to deaden old adhesive residual tack and seal dusty or porous plywood and
concrete floors. Note: EverSeal must be applied to bare, clean concrete to serve as moisture
barrier.
For this warranty to apply, documentation of temperature and humidity at the time of
application must be sent to Lees Technical Services. If using EverSeal to correct a moisture
problem, ealdum chloride test results must also be sent to Lees Technical Services, along
with the sine of the installation and the amount of EverSeal purchased, either through a
copy of the invoice trom Lees Customer Service or documentation from the Lees authorized
dealer where the EverSeal was purchased. Lees also requires documentation of purchase of
a Lees Adhesive. Send documentation to: Lees Technical Service, Attn: EverSeal. Warranty,
65 East Ferguson Avenue, Wood River, IL 62095.
This warranty covers full replacement of material and reasonable labor costs for the affected
installation. This guarantee only covers those installations where a Lees' adhesive product and
Lees' floor covering are being installed over Lees EverSeal. The Lees warranty does not cover
those installations where Lees floor coverings and Lees' adhesives are net used.
CAUTION! EverSeal cannot be used over chemically cleaned sub -floors.

Surface Preparation

EXPLANATION:
The fire station has vinyl tile, rubber mats, and carpet installed as flooring throughout the meeting rooms
and living areas. These flooring items are glued directly on the slab on grade. This area is a total of 6,100
square feet. In order to install these products and warranty the installation it is necessary to have vapor
emissions below industry standard levels. Vapor emissions are determined through an ASTM test known
as Calcium Chloride emission testing. Calcium chloride vapor emissions can often be seen on concrete in
the form of efflorescence — a loose white chalky substance on the concrete.
The specifications do not require that a vapor barrier be installed. When the project was designed it was
believed that the anticipated construction schedule would allow the slab on grade to be poured early, and
it would dry to an acceptable level by the time these floorings were installed. Paying for a vapor barrier
was thought to be an unnecessary expense.
The slab was poured in August of 2004 as anticipated, however, due to the topography of the site and the
record amount of rainfall that has occurred this year; the slab has not dried as anticipated. in order to
properly install these flooring materials it is necessary to install this vapor barrier. This barrier will ensure
the integrity of the concrete surface, prevent the efflorescence, and limit any degradation of the glue and
the flooring product installed.

WHEREAS, due to the topography of the site and the record amount of
rainfall this year, the 6,100 square foot slab of the building has not dried as anticipated,
requiring the installation of a vapor sealer on the slab grade of the building before the
flooring materials are laid.
NOW, THEREFORE, BE IT RESOLVED that the City Council of the City
of National City hereby authorizes the City Engineer to execute Change Order No. 18
with Marcotte & Heames Builders, Inc. in the total amount of $8,392 for the installation of
a vapor sealer on the slab grade of the building for the construction of the fire station
project.

LEES EVERSEAL
Lees Universal Concrete and Flora Sealer is a latex -based sealer designed for ensuring the
adhesive bond for installations where potential problems exist. EverSeal has excellent resistance
to moisture penetration, adhesive bleed -through from latex -haled adhesives, and dust and oil
contamination. It is ideal for use in sensitive environments; it has very low odor and low VOCs
(volatile organic compounds) and contains and antimicrobial agent. EverSeal is designed for use
with all Lees' installation products' Lccs Wet Set Adhesive. Lees Squared Pressure Sensitive
Adhesive, Lees Thermotape, and Lees NeoFloor Adhesive. EverSeal is packaged in four -gallon
containers; spread rate is approximately 40 yards per gallon per coat.
One Coat of EverSeal Is Recommended for the Following Conditions:
Busty subfloors - EverSeal seals out dusty. dirty subfloors. providing a clean substrate an which
to install any of the Lees adhesives and carpet products. EverSeal will also seal Cryperete patches
and oil contamination over wood subfloors.
Porous Suhfloors - EverSeal will seal porous subfloors, preventing the loss of adhesive into the
subfloor. Porous subfloors can include unsealed concrete or hardwood, or plywood subfloors.
EverSeal will also seal concrete patching compounds.
Latex -based Adhesives - Lees EverSeal is guaranteed to seal any existing latex adhesive residue
that may remain on the subfloor. EverSeal serves as a barrier between the old and new adhesives
to prevent bleed -through or reactions with the new adhesive. EverSeal will also deaden residual
adhesive tack to provide easier installation of the new carpet. NOTE: Lees EverSeal is not
warranted for use over cut -back or black solvent -based adhesives. To seal old cut -hack or black
solver( -based adhesives, use Lees SureSeal ardhfloor seater.
Two Coats of EverSeal Are Required for the Following Conditions:
Elevated pH (Alkalinity) Levels — EverSeal will provide a harrier coat to prevent alkaline salts
in the slab from reacting with the adhesive_ If the pH level of the subfloor is between 9 and 14,
mop the fkx,r with a vinegar and water ,solution (eight ounces of vinegar to one gallon of water) to
remove alkalinity on the surface of the slab. Allow this solution to dry completely, then use two
coats of Lees EverSeal to seal the surface of the concrete (see EverSeaJ installation instructions).

Elevated Moisture Levels - EverSeal is recommended for any subfloor with a calcium chloride
test result up to 8 pounds ofmnisrure per 1,000 square feet per 24 hours when used with Lees Wet
Set Adhesive. Installations using pressure sensitive adhesives, tape systems, or any of Lees' other
adhesive systems can be protected up to 8 pounds of moisture when using Lees EverSeal.
However. Lees Technical Service must he consulted if the moisture reading is above 5 pounds for
these types of installations. Lees will assist the contractor in determining the best application
method based on site conditions. Tf two coats of EverSeal are applied, with both coats receiving
24 hours of curing time, EverSeal is guaranteed for the lifetime of the carpet as a moisture harrier
between the subfloor and Lees adhesives. Please note that the calcium chloride test measures
moisture, not hydrostatic pressure. EverSeal is not guaranteed against hydmstatic pressure.
Warranty
When used according to instructions and in conjunction with Lees' carpets and adhesives, the
Less Lifetime Warranty extends to the adhesive bond of the carpet to the subfloor. This warranty
covers EvcrSeal's ability seal out moisture emission up to 8 pounds per 1,000 square feet per 24
hours as measured by a calcium chloride test (ASTM F 1869-98) when used with Lees Adhesives.
(For adhesives other than Lets Wet Set, crmtact Lecs Technical Service if the moisture reading is
between 5 and 8 to determine the best application method.) If the moisture level is over the
recommended limits, other means roust be employed to curtail .moisture emissions before
installation. NOTE: Lees Technical Service must be consulted if the moisture reading is
above 5 pounds for these types of installations. Lees will assist the contractor in determining
the test application method based on sight conditions.
EverSeal is ai.so warranted to also stop alkaline salts from rising to the surface of the slab,
providing the slab is cleaned with the vinegar and water solution before application. Also,
EverSeal is guaranteed to deaden old adhesive residual tack and seal dusty or porous plywood and
concrete floors. Note: EverScal mast be applied to bare, clean concrete to serve as moisture
barrier.
For this warranty to apply, documentation of temperature and hnmldity et the time of
application must be sent to Lees Technical Services. If using EverSeal to correct a moisture
problem, calcium chloride test results must also be sent to Lees Technical Services, along
with the sine of the installation and the amount of EverSeal purchased, either through a
copy of the invoice from Lees Customer Service or documentation from the Lees authorized
dealer where the EverSeal was purchased. Lees also .requires documentation of purchase of

This warranty covers full replacement of material and reasonable labor costs for the affected
installation. This guarantee only covers those installations where a Lees' adhesive product and

How to Apply
Surface Preparation
I. Ensure that floors ere stnrcturelly sound and
fully cured a minimum of 28 days.
2. Repair concrete end install joint sealants and
fillers as necessary. the patching materials as

3. Mechanical surface profiling is the preferred
floor preparation method for both new aid existing
