HomeMy WebLinkAbout2007 CON CDC Healey Construction - Change Order #3 Foodland MarketCHANGE ORDER CIO Number 3 To Healey Construction Co.: California C/O Initiation Date: January 11, 2007 Project: Foodland Market Project Specification No.:06-04 A Change Order is hereby recommended for the following work in the amount identified in this document: DESCRIPTION OF CHANGE: Provide an asphalt overlay (1-1/2 inch to 4 inches) to eliminate the sump in the existing parking lot and to eliminate the curb created adjacent to the new ADA walkway and settled parking area. The asphalt work will be done in accordance to the Standard Specifications for Public Works and as attached. REASON FOR CHANGE: The existing asphalt parking lot has settled over time and created a sump. In order to comply with ADA walkway regulations the new concrete walkway from the street to the front entrance creates a 2 inch to 4 inch lip over the settled parking lot area. This change order will provide for placement of asphalt in an area of approximately 3,000 sq. ft to accommodate the difference in elevation and allow for drainage of the parking lot. A short work plan is attached. COST RECONCILIATION; Costs were reviewed and negotiated to the satisfaction of the City. The work will he compensated through the project budget.(Sec attached detailed breakdown). The asphalt overlay area is approximately 2,600 sq. ft to eliminate the 3 to 4 inch curb lip on the new ADA walkway created by the existing settled AC pavement approximatelyl5- 20 tons of asphalt will be required. Prior to the application of the tack coat dust and debris will be removed and the existing asphalt cleaned. Tack coat will be grade SS-1 h and applied at a rate of 0.05 to 0.10 gallons per square yard. Asphalt concrete shall consist of one or more courses of asphalt, Type III-C2-AR-4000 or superpave PG-64-10, 3/8-inch nominal size mix. A level course and leveling of spot repairs will be provided to allow for a smooth surface overlay. The temperature of the asphalt mixture directly behind the paving machine will not be lower then 270 degrees F. The compaction after rolling shall average 95 percent of the density as determined by Ca. Test 375 but in no case shall be less then 92 percent. Paved areas not subject to vehicular traffic may be compacted to 90 percent of Ca. Test 304. The asphalt overlay will provide positive drainage for the parking area.
