A Change Order is hereby recommended for the following work in the amount identified below:
Description of Change in the Contract
See Attachment "A".
Cost Reconciliation
This change in the contract was reviewed and negotiated by and between the City Representatives and the
Contractor and represents the entire agreement for the work described herein and includes all costs
associated with the work including but not limited to manpower, equipment, labor, fees, taxes, insurance,
bonding, overhead, profit, etc. and there will be no other compensation. This Change Order hereby
incorporates the original contract and all its referenced documents' terms, conditions, and plans for the work as
provided by the City.
Justification for Change in Contract
The surface of the basketball court at Kimball Park is in need of IMMEDIATE repair. The base contract is
primarily for work in Kimball Park and other parks and, therefore is substantially related to the base contract
scope of work and will present a substantial cost savings to expedite this work through the change order
process.

Change Order #2, Attachment "A"
Change In Contract: Repair and resurface the basketball court at Kimball Park.
Contractor shall:
1. Waterblast the existing surface using a 3500 to 5000 psi hydroblaster to remove dirt and loose
surface material.
2. Remove any bubbling or peeling of surfaces.
3.Patch all visible pop -outs and spalls using court patch grinder, grind or sand flush with existing
slab.
4. Fill all cracks wide enough to accept court patch binder, grind or sand flush with slab.
5. Surface playing area with " Latex-ite Acrylic System" for true ball bounce:
a. Clean and prepare surface.
b. Apply one coat of acrylic resurfacer, producing a smooth, even, uniform finish.
c. Apply one coat of sand filled acrylic color for court speed medium.
d. Seal surface with one coat of unsanded acrylic color (to match existing color scheme).
e. Stripe the court as per current USTA specifications in pure white acrylic.
6. Work for this change order will commence Friday October 3rd and be completed no later than
Tuesday October 7th.
