A Change Order is hereby recommended for the following work in the amount identified below:
Description of Change to the Contract:
This Contract Change Order is for raising two existing Electrical outlet in Public Works Lunch room (Schedule C)
located at 2100 Hoover Ave. The new location will be installed above the new kitchen cabinets that are being
installed as a part of the Schedule C work. The work shall be per Jim Ridley Email dated April 29, 2010 and per
Specs 09-04 (see attached Email).
Cost Reconciliation:
This change in the contract was reviewed and negotiated by and between the City Representatives and the
Contractor and represents the entire agreement for the work described herein and includes all costs associated
with the work including but not limited to manpower, equipment, labor, fees, taxes, insurance, bonding,
overhead, profit, etc. and there will be no other compensation. This Change Order hereby incorporates the
original contract and all its referenced documents' terms and conditions.
Justification for Change:
The Contractor's Electrical work proposal is for Schedule C work and is required because the new kitchen
cabinets will cover the existing electrical outlets therefore the outlets needs to be relocated 2" above the lower
cabinets. The proposed electrical Change Order amount is for $525 and is within the available budget.

Subject: Public Works Kitchen Schedule
Hi Bruce and Tony- Please be advised that work on the Public works kitchen can not start until Tuesday May 4,
2010. Because we had not provided a schedule, Public Works can not rearrange its manpower needs quicker
than that. On the issue of the electrical and plumbing fixtures on the vacant wall where the new cabinets are to be
installed we would like you to provide us with a Change order request for that work. The Change Order should
include costs for these items based on labor and materials:
1. Provide a hole in the cabinet back panel the circumference of the plumbing fixture cover plate.
2. Drill a properly sized whole in the side panel of the cabinet for the water supply line to the ice machine to
pass through from inside the cabinet.
3. Provide a hole in the back panel of the left side cabinet properly sized for an electrical cover plate.
4. Cut out a sufficient amount of drywall between the stud bay up to the appropriate counter height for an
outlet.
5. Use the existing outlet box as a junction box and provide a cover plate.
6. Extend the wiring in the stud bay up to the new outlet location and install a new box, outlet and cover plate.
7. Replace the drywall and tape the seams.
8. Provide a hole in the back panel of the right side cabinet properly sized for an electrical cover plate.
9. Cut out a sufficient amount of drywall between the stud bay up to the appropriate counter height for an
outlet.
10. Use the existing outlet box as a junction box and provide a cover plate.
11. Extend the wiring up the stud bay to the new outlet location and install a new box, outlet and cover plate.
12. Replace the drywall and tape the seams.
The cutouts should allow the cabinets to sit flush against the wall. I realize that there are a lot of line items for not
much work but I wanted to be clear on the intention.
So that we can keep everyone affected by this work appraised of the situation, would you provide us with a basic
work schedule for the Public Works kitchen remodel? I assume that the carpet choice in the Admin Building will
affect your schedule there so when that choice has been made we will need a schedule for that work also. If your
plan is to work in the Admin kitchen first and then do the carpet room then please let us know that schedule also.
