A Change Order is hereby recommended for the following work in the amount identified below:
Description of Change in the Contract
Consultant conducted soil smapling nad prepard a summary report in conjunction with the violation at the
National City Municipal Golf course.
Cost Reconciliation
This change in the contract was reviewed and negotiated by and between the City Representatives and
the Contractor and represents the entire agreement for the work described herein and includes all costs
associated with the work including but not limited to manpower, equipment, labor, fees, taxes, insurance,
bonding, overhead, profit, etc. and there will be no other compensation. This Change Order hereby
incorporates the original contract and all its referenced documents' terms, conditions, and plans for the
work as provided by the City.
Justification for Change in Contract
The cost of the soil sampling was reimbursed to the City by the violator. The scope of work was not part
of the orginal contract and therefore left the contract short of funds to complete the NPDES program.

Re. Change Order Request for conducting Soil Sampling for National City Golf
Course
Dear Barby,
Per your request, D-Max conducted soil sampling and prepared a summary report for the
National City Golf Course during July 2009. The cost of this task was $3,453 which was
included in our invoice No. 2162 dated August 11, 2009. Since this service was above and
beyond the scope of services of our current Contract Agreement with the City of National City
for fiscal year 2009-2010, we hereby request that the Purchase Order No. 72817 be adjusted
to reflect this amount.

Re. Change Order Request for Assisting in Unfunded Mandate Test Claim for the City
of National City
Dear Barby,
Per your request, D-MAx Engineering, Inc. assisted the City in its unfunded mandate test
claim by providing cost information during December 2009 and February 2010. The cost of
this service was $661.25, which was included in our invoices No. 2216 and 3330 dated
January 18, 2010 and March 26, 2010 respectively. Since this service was above and
beyond the scope of services of our current Contract Agreement with the City of National City
for fiscal year 2009-2010, we hereby request that the Purchase Order No. 72817 be adjusted
to reflect this additional amount.
