CONTRACT AGREEMENT PUBLIC WORKS
MONITORING WELL INSTALLATION
THIS AGREEMENT, made this 9111 day of February, 2011 by and between the
City of National City, California, hereinafter referred to as "Owner", acting herein
through its City Engineer, and GeoHydrologic Consultants, Inc. a corporation
doing business as a corporation in the City of National City, County of San Diego
and State of California hereinafter referred to as "Contractor".
WITNESSETH: That for and in consideration of the payments and agreements
hereinafter mentioned to be made and performed by the OWNER the
CONTRACTOR hereby agrees with the OWNER to commence and complete the
construction described as follows:
WORK TO BE DONE: Removal of two existing well boxes and installation of two
new well boxes located within the OWNER Street Right -of -Way and all other
work as described in Exhibit A.
Herein called the Project, for the sum of One Thousand Five Hundred Dollars
($1,500.00) and all extra work in connection therewith, under the terms as stated
in the attached proposal, General and Special Provisions and plans (see Exhibit
A); and at his (its or their) own proper cost and expense to furnish all the
materials, supplies, machinery, equipment, tools, superintendents, labor,
insurance, sales tax and other accessories and necessary to complete the said
Project in accordance with the conditions and prices stated in the bidder's
proposal (see Exhibit A). The General Provisions and Special Provisions of this
contract and plans, which include all maps, plats, blueprints other drawing, and
printed or written explanatory matter thereof, the specifications and Contract
Documents therefore, all which are made a part hereof and collectively evidence
and constitute the Contract.
The CONTRACTOR hereby agrees to commence work under this Contract on or
before February 9, 201 1 and to fully complete the Project within 5 working days
thereafter. The CONTRACTOR further agrees to pay, as liquidated damages,
the sum of $100.00 for each consecutive calendar day that he/she is in excess of
the time specified for completion.

The OWNER agrees to pay the Contractor in current funds for the performance
of the Contract, subject to additions and/or deductions, as provided in the
General Provisions of the Contract and to make payments on account thereof.
IN WITNESS WHEREOF, the parties to these presents have executed this
Contract in 2 counterparts, each of which shall be deemed an original, the year
and day first mentioned above.

INFORMAL CONTRACT
GENERAL PROVISIONS
PROJECT: MONITORING WELL INSTALLATION
1. REFERENCE SPECIFICATIONS: Except as otherwise noted as specified
the work shall be done in accordance with the following reference
drawings
1. STANDARD DRAWINGS
San Diego Regional Standard Drawings, "GreenBook" Standard
Specifications for Public Works Construction and City of National
City Standard Drawings.
BUSINESS LICENSE: Contractor, subcontractor and suppliers who do not
have a current City of National City Business License will be required to
obtain the appropriate license from the City Business License Inspector
before a contract can be executed. Said Business License is not required
from a supplier who delivers material and/or equipment with the
Contractor's subcontractor's vehicle or with a common carrier.
Contact the City Business License Inspector at (619) 336-4341 for further
information regarding the license fees, application, etc.
3. INSURANCE
A. Liability Insurance — Contractor shall furnish the City a policy or
Certificate of Liability in accordance with Section 7-3 of the
Standard Specification. The policy or certificate shall include:
a. Bodily Injury $150,000.00 each person
$300,000.00 each occurrence
$300,000.00 aggregate products and completed
operation
b. Property Damage: $100,000.00 each occurrence
$150,000.00 Aggregate
c. The City shall be named insured or is named as additional
insured with the Contractor.
d. In the event of expiration or proposed cancellation of such
policies the City shall be notified, but in no event less than
30 days before expiration or cancellation is effective.
B. Workers Compensation Insurance: Contractor shall comply with
Section 3700 and 3800 of the Labor Code.
4. The contractor shall provide the payment bond for 100% of total
construction cost.
5. Underground Service Alert (Dig Alert).
Section 4216/4217 of the Government Code requires a Dig Alert
Identification number be issued before a "Permit to Excavate" will be valid.
For your Dig Alert I.D. Number, call Underground Alert Toll Free 1-800-
227-2600 two working days before you dig.
6. The Contractor is required to comply with the Section 1771 of the Labor
Code regarding the General Prevailing Wage rates.
7 The Contractor shall promptly remove all debris and salvable material by
hauling. The burning of debris from the work site is not permitted.
8. The Contractor shall so conduct his operations as to offer the least
obstruction and inconvenience to the public.
9. The Contractor shall so conduct his operations as to offer the least
possible obstructing and inconvenience to the public and he shall have
construction no greater length or amount of work than he can prosecute
with due regards to the right of the people.
10. Construction operation shall be conducted in such a manner as to cause
as little inconvenience as possible to abutting property owners. The
Contractor shall notify all residents in the immediate area directly affected
by his work 48 hours before commencing the work.
11. Any public improvements project not completed and cleaned up within 10
days of the inspection of the forms is subject to being required to complete
the work including overhead, plus a surcharge of 50%.
12. Work is to be performed only during the hours between 7:00 a.m. and 5:00
p.m. Monday through Friday unless special permission has been obtained
from the City Engineer.
13. No material shall be transported to any other site in National City without
obtaining an additional permit therefore.
14. City "STREETS" shall be kept clear of spillage, dust, dirt, mud, etc.
15. Improvements installed by Contractor which do not meet City
requirements as to design, location, or construction shall be modified to
meet City requirements, or removed if directed to so by the City Engineer.
If applicant fails to act upon notice from the City Engineer, the City can act
for applicant and charge applicant for correction of remedial work.
16. The Contractor shall obtain all necessary permits.
SCOPE OF WORK
The contractor shall perform the following;
1. Obtain City of National City construction permit (no fee)
2. Breakout, removal and disposal of two damaged well boxes.
3. Installation of two new well boxes to include the following,
A. Traffic Control Plan and Traffic Control setup
B. Equipment setup.
C. Purchase and installation of new well boxes.
D. If necessary water inside the well boxes will be collected and placed in
drums
E. Properly dispose of well box materials and water.
4. Provide GeoHydrologic Consultant Labor for well box installation oversight
5. Provide Well box removal and installation report to the City Engineer
6. Installation of new well boxes shall be to the satisfaction of the County of San
Diego Local Enforcement Agency
SCHEDULE OF WORK
1. The contractor shall begin well work within 2 days from the date of the
agreement.
2. Well box removal and installation shall be completed within 5 days from the
start of work.
3. The length of this agreement is for 10 days.
Bi-weekly progress meetings shall be held between the City Project Coordinator, the
Contractors Project Director and the City Engineer or her representative.
