CHANGE ORDER NO. 5 2011 JUL I LI P 3: jfille 22, 2011 Reason/Purpose for this Change Order: The ADA improvements for the parks are complete for this phase. Any additional ADA improvements would require more funds than are remaining and available for this budget. This reduction is $9,848.00 and represents approximately 4% of the contract total. The City of National City and Contractor agree to the following change to the original Contract: The Contractor and the City agree to reduce various line items of work as represented on the attached spreadsheet and that there will be no additional compensation or credits for this reduction in line item work. Additionally, Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
