DRAINAGE PROJECT, FY 11-12 Specification No. 11-07 Contractor: Palm Engineering and Construction, Inc. CHANGE ORDER NO. 1 May 8, 2012 Reason/Purpose for this Change Order: After potholing utilities on D Street is was determined that there is an existing high pressure gas main at 5th Street that will not have enough cover once the work is complete. SDG&E has investigated and determined that they will allow a deviation if concrete protection is provided over the top of the gas main in conformance with their attached standard drawingfor any location with less than 32" of cover. The City of National City and Contractor agree to the following change to the original Contract: The Contractor agrees to install a concrete cover over the top of the existing high pressure gas main on D Street at 5th Street in conformance with the SDG&E specifications, procedures and the attached drawing where there will be less than 32" of cover from the final surface improvements. Contractor shall coordinate all work with SDG&E and document ail work and utilities on their As -Built drawings. The price shall be $30 per lineal foot of I concrete cap place and shall include the cost of all work, overhead and profit involved in placing the concrete cap, and preparing each location to accept the final improvements. The estimated amount of lineal footage needed is 60 feet. As the gas main is exposed the actual footage shall be determined by the Contractor, SDG&E and the inspector. Another change order shall be issued to adjust the price of this extra work for actual footage placed. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Additionally, Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
