Reason/Purpose for this Change Order: As of the date of this change order Media 3 is no longer a part of this utility undergrounding project. Currently 390 lineal feet of 2" conduit has been installed for Media 3. The City of National City and Contractor agree to the following change to the original Contract: ;The Contractor and the City agree to delete the following line items: COMMUNICATIONS (MEDIA 3) UNDERGROUNDING, #41(S) 60 If of 3" conduit at $2.46/If=$147.60, #42 (S)18,360 If of 2" conduit at $1.66/If=$30,477.60, #43 (S) 15 SGL's at 631.30 each=$9,469.50 and #44(S) 9 DBL's at 732.95 each=$6,596.55'' ;for a total reduction in the contract amount of $46,691.25. These deductions do not effect any other line items since they were only for Media 3 conduit in joint trenches and Media appurtenances. CHANGE ORDER NO. 2 June 28, 2012 Reason/Purpose for this Change Order: IThe purpose of this Change Order is provide undergrounding of Access Cable utilities by furnishing and installing :2" CATV conduit in a trench under D Avenue from the south side of the 8th Street to the north side of 8th Street ;which was not included in the original plans. See Attachment "A", attached hereto and incorporated herein, for a visual plan of the works to be provided by this Change Order. The City of National City and Contractor agree to the following change to the original Contract: The Contractor and the City agree that the Contractor shall furnish and install 305 lineal feet of CATV 2" Conduit 'sin the D Avenue trench from the south side of 8th Street to the north side of 8th Street. 290 feet of this conduit shall be furnished and installed at the same price and conditions as line item 38(S) of the original contract and 15 feet of trench on D Avenue north of 8th Street shall be at same price and conditions as Line Item 36(S) of the original contract. Compensate for extra traffic control, curb, gutter and sidewalk shall be per Attachment "B", attached hereto and incorporated herein. The Contractor and City agree that all costs for the construction of the above works are included of the price of this Change Order.
