HomeMy WebLinkAbout2012 CON Black Rock Construction - Change Order #2 - Concrete Improvements, FY 11-12ems+» CALIFORNIA %A.1ioN L arry .. rNCOR.pORATED CONCRETE IMPROVEMENTS PROJECT FY 11-12 Specification #11-06 Contractor: Black Rock Construction Company Reason/Purpose for this Change Order: Removal and replacement of the existing concrete improvements is necessary due to the damaged condition of the existing driveway at the National City Fire Station located on C Avenue, north of 16th Street. CHANGE ORDER NO. 2 February 6, 2012 The City of National City and Contractor agree to the following change to the original Contract: The Contractor agrees to remove the driveway and additionally the subgrade to an 8 inch depth and reinstall the driveway section with 8" of crushed miscellaneous base, 8" of 5000 psi (7.5-sack) concrete with #5 rebar, 12" on center both directions, at the National City Fire Station located on C Avenue just north of 16th Street. Rebar shall be placed between 2"-3" above the subgrade for the driveway . The Contractor shall repair asphalt concrete that was damaged or removed to necessitate the removal and installation of the new driveway. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Additionally, Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
