: Reason/Purpose for this Change Order:
!During the trenching for the underground wet utilities on 8th Street and Highland Avenue, the Contractor
`encountered additional unmarked abandoned utility conduits that were not shown as existing on the original
plans. Due to the location of the conduits, the Contractor was unable to excavate using an Excavator, as
calculated in its bid price, and was required to excavate this location by hand resulting in additional labor costs.
The City of National City and Contractor agree to the following change to the original Contract:
'The City shall pay the Contractor for the additional labor outlined above in the amount recorded and confirmed by
the field Inspector. The worker classifications and hours worked for the additional labor are specified on
:Attachment "A", which is attached hereto and incorporated herein. The Contractor and City agree that all costs
for the additional labor is included in the price of this Change Order.

;Reason/Purpose for this Change Order:
`On June 1, 2012, while trenching for underground wet utilities on 8th Street, the Contractor encountered an
)angled sewer lateral that did not comply with City's sewer lateral installation standard requiring laterals to be
installed in a perpendicular manner from the sewer main to the property connection point. To comply with the
City's sewer lateral installation standard, the Contractor was required to relocate the angled lateral to a different
:location than that which resulted in additional costs for labor, materials, and equipment.

'The City shall pay the Contractor for the additional labor, equipment, and materials in the amount recorded and
:confirmed by the field Inspector. The worker classifications and hours worked for the additional labor are
'specified on Attachment "A", which is attached hereto and incorporated herein. The Contractor and City agree
that all costs for the additional labor, equipment, and materials is included in the price of this Change Order.

Reason/Purpose
for this Change Order:
On June 25th, July 2nd, and July 3rd, while installing sewer laterals on 8th Street, the Contractor encountered
angled sewer laterals that did not comply with City's sewer lateral installation standard requiring laterals to be
installed in a perpendicular manner from the sewer main to the property connection point. To comply with the
City's sewer lateral installation standard, the Contractor was required to pothole at multiple locations to determine,',
the location where each lateral crossed the private property line.
The City of National City and Contractor agree to the following change to the original Contract:
'The City shall pay the Contractor for the additional labor, equipment, and materials in the amount recorded and
confirmed by the field Inspector. The worker classifications and hours worked for the additional labor are
specified on Attachment "A", which is attached hereto and incorporated herein. The Contractor and City agree
