Reason/Purpose for this Change Order:
In accordance with Change Order No. 1, the Contractor, SDG&E, and the inspector determined in the field that
one hundred lineal feet of concrete cover was required for the uncovered gas main on D Avenue and 5th Street.
:As Change Order No. 1 provided only for sixty (60) lineal feet of concrete cover, this Change Order is necessary
to reflect the additional 40 lineal feet of concrete cover required to be installed.
The City of National City and Contractor agree to the following change to the original Contract:
The Contractor agrees to install an additional 40 feet of concrete cover over the top of the existing high pressure
gas main on D Avenue at 5th Street in conformance with the SDG&E specifications, procedures and the
drawings attached to Change Order No. 1. The Contractor shall coordinate all work with SDG&E and document
all work and utilities on their As -Built drawings. The price shall be $30 per lineal foot of concrete cover placed
and shall include the costs for all work, overhead and profit involved in placing the concrete cap and preparing
each location to accept the final improvements.

By signing this Change Order the Contractor confirms that he/she is completely familiar with its terms and conditions and has fairly negotiated
its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to
provide a complete work as described above at that the price stated herein. Additionally, Contractor confirms that the price is fair and complete
and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees,
bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents
are incorporated herein.
