The City of National City and Contractor agree to the following change to the orig
The Contractor shall backfill and repair 40 lineal feet of Mainline Joint Trench at D Avenue, south of 8th Street.
The Contractor shall backfill and repair the Mainline Joint Trench at the same price and subject to the same
terms and conditions as line item 11(S) of the original contract. The Contractor shall provide saw cutting, traffic
,control, and install AC Pavement as required for the backfill and repair of the additional Mainline Joint Trench and:
;laterals. The Contractor shall be compensated for these additional items in accordance with negotiated prices for,
these items of work, as stated on Attachment "A" to this Change Order, attached hereto and incorporated herein.
The Contractor and City agree that all costs for the construction of the above works are included in the price of
this Change Order.

Reason/Purpose for this Change Order:
After approval, COX Communications representatives decided to install higher capcity Cable TV vaults than
shown on the original plans. COX Communications agrees to compensate the City of National City for the full
price of this Change Order.
CHANGE ORDER NO. 8

[The City of National City and Contractor agree to the following change to the original Contract:
The Contractor shall trench thirty (30) lineal feet for Cable TV conduit, furnish and install 180 lineal feet of Cable
TV (1) 2" conduit, furnish and install seven (7) Cable TV Vaults (2'x3'x3'), remove and replace 25 square feet of
brick sidewalk, and remove and replace pavers. Contractor shall trench for CATV at the same price and subject
to the same terms and conditions as line item 36(S) of the original contract. Contractor shall furnish and install
ECATV 2" Conduit at the same price and subject to the same terms and conditions as line item 38(S) of the
original contract. The Contractor shall be compensated in accordance with other prices for the other items of
work, as stated on Attachment "A", attached hereto and incorporated herein. The Contractor agrees that all
costs for the construction of the above works are included in the price of this Change Order.

Reason/Purpose for this Change Order:
While excavating in an alley way to install surface improvements adjacent to 8th Street between B Avenue and C
;Avenue, the Contractor encountered unsuitable subgrade material. The Contractor and Inspector agreed in the
jfield that the existing subgrade material was unsuitable and the Contractor was instructed to remove and dispose
of the unsuitable subgrade material and import suitable subgrade.
The City of National City and Contractor agree to the following change to the original Contract:
;The Contractor removed and disposed of unsuitable subgrade material and imported 22.96 TONS of suitable
Isubgrade for the alley way adjacent to 8th Street between B Avenue and C Avenue. The work performed and the.
;quantities placed were verified by the Inspector in the field. The Contractor shall be compensated for these items'
of work in accordance with the unit bid price for Line Item 70, $78.56 per TON. The calculation for compensation

While excavating near manhole 265C, the Contractor encountered an electric conduit that was not shown as
:existing on the original plans. The Contractor was required to break up and remove the conduit in order to
continue excavating for the installation of manhole 265C. This resulted in additional costs to the Contractor for
labor, equipment, and materials.

On July 17, 2012, the Field Inspector advised that manhole #265C, that was to be placed in the sidewal,k should I'

Reason/Purpose for this Change Order:
On July 25, 2012, while excavating for manhole #264 on 8th Street, the Contractor encountered a concrete
encased 18" drop sewer main on the outside of the manhole that was not called out on the original plans. The
,Contractor was required to excavate additional soil to place larger box shoring shield and to remove the encased
pipe in order to continue removing and installing manhole # 264. This resulted in additional costs for labor,
[equipment, and materials.
The City of National City and Contractor agree to the following change to the original Contract:
The City shall pay the Contractor for the additional labor, equipment, and materials in the amount recorded and
confirmed by the field Inspector. The worker classifications and hours worked for the additional labor are

On July 26, 2012, the Contractor continued breaking and removing the drop main outside mahole #264 and
encountered an unmarked drop main outside manhole #265B. The Contractor began removing the concrete
drop sewer main. This resulted in additional costs to the Contractor for labor, equipment, and materials.

Reason/Purpose for this Change Order:
On July 30, 2012, the Contractor cutout of manhole #264 riser to accomodate the drop main re -installation and
set 18" SDR 35 PVC sewer main drop inside manhole #264. The Contractor encountered an unknown old
concrete sewer main approximately 25' east of manhole #265B, just below the flow line grade of the new pipe to
be installed, which discharged large amounts of fluid. City crews could not identify this pipe. It was determined
that this pipe was not live and was to be removed and disposed of. These items required the Contractor to
remove additional oversaturated subgrade and pump out additional fluid. This resulted in additional costs to the
Contractor for labor, equipment, and materials.
The City of National City and Contractor agree to the following change to the original Contract:
The City shall pay the Contractor for the additional labor, equipment, and materials in the amount recorded and
'confirmed by the field Inspector. The worker classifications and hours worked for the additional labor are

On August 8, 2012, while excavating on 8th Street, it was deteremined by the Field Engineer that teh Contractor
could stop removing the an unknown existing concrete sewer main at the manhole located at station 13+10 at a
grade below our new manhole. The Contractor installed a concrete plug in order to continue with the installation
of the sewer main and manholes. This resulted in additional costs to the Contractor for labor, equipmnet, and

Reason/Purpose for this Change Order:
!While excavating for the installation of sewer on 8th Street from approximately 25 feet east of manhole 265E to
:the manhole at Roosevelt, the Contractor encountered an old abandoned concrete sewer line with oversaturated,
'unsuitable subgrade material at the flow line of the new sewer main that was progressively getting deeper. At
,the direction of the Field Engineer, the Contractor removed and disposed of the old sewer main and the
;unsuitable material and replaced it with 82 TONS of suitable import material. This removal and replacement of
!unsuitable subgrade is of similar character to the requirements of line item #70.
The City of National City and Contractor agree to the following change to the original Contract:
:The Contractor agreed to remove and dispose of the old existing sewer and the unsuitable subgrade material
land replace it with 82 TONS of suitable subgrade as directed by the Field Engineer. Although there is no line
item for this work, the Contractor and City agree that the Contractor will be compensated at the same rate as
Line Item #70, $78.56 per TON, as this item is substantially similar in nature and scope to the work actually
!completed. The itemized breakdown for compensation under this Change Order is found on Attachment "A",
which is attached hereto and incorporated herein. The Contractor and City agree that all costs for this work is
gincluded in the price of this Change Order.
