Reason/Purpose for this Change Order: Existing D-25 requried repair on the North Side of Plaza Boulevard between D Avenue and Highland. The City of National City and Contractor agree to the following change to the original Contract: The contractor shall remove and repalce the top portion of the existing D-25 on the North Side of Plaza Boulevard between D Avenue and Highland. The lump sum change order price shall include all sawcut, demolition, excavation, hauling, disposal, setup, drive time, and traffic control required to complete the work. The Contractor shall be compensated for these additional items in accordance with negotiated prices for these items of work, as stated on Attachment "A", attached hereto and incorporated herein. The Contractor and City agree that all costs for the construction of the above work is included of the price of this Change Order.
