CHANGE ORDER NO. 1 January 21, 2014 Reason/Purpose for this Change Order: After inspection of the Shop Building on site, it was determined that the door, frame and hardware located at the South East Corner of the building is in poor condition. Additionally, it was also determined that Fire Labels on the exterior doors and frames on the east side of the Shop Building must be placed in order to comply to the General Requirements of Fire Department of the city. The City of National City and Contractor agree to the following change to the original Contract: The City and the Contractor agree that the Contractor shall remove and replace the damaged door, frame and hardware of the Shop Building located in 1726 Wilson Avenue site as well as the installation of the Fire Labels on Exterior Doors and Frames on the East side of the Shop Building. The lump sum change order price includes all labor, materials, and equipment required to complete this work. The agreed upon price for all costs associated with this work is $2,465.03 in total. CHANGE ORDER NO. 2 February 3, 2014 Reason/Purpose for this Change Order. Upon observation, it was determined that the interior walls of the Office Building will need to be repainted. This change order entails the cost for the work for painting the walls. The City of National City and Contractor agree to the following change to the original Contract: The City and the Contractor agree that the Contractor shall paint and make minor wall patch work for the interior of the Office Building on site. The work for this Change Order will include the preparation of the walls (minor patch work). Then, pretreat the walls for painting in accordance with the paint manufacturer's requirements and apply paint for full coverage. Reason/Purpose for this Change Order: During the initial trench work for the 1726 Wilson Avenue site, the contractor has discovered two 17'x17'x24' receptors in the existing floor drains in the shop and wash rack area of the Maintenance Building. As seen per attached Exhibit 1, the receptors contain unknown material that was determined to be 25-50% Oil and 50-75% water by ACT (Advanced Chemical Transport) after testing. ACT will then remove and transport the tanks containing the material to an approved disposal location. This change includes the cost for generating a waste profile for the material as well as its safe transportation to a disposal site. CHANGE ORDER NO. 3 February 3, 2014 The City of National City and Contractor agree to the following change to the original Contract: The City and the Contractor agree that the Subcontractor shall generate an accurate waste profile for the discovered fluid material in the Shop Building. This Change Order will also include the cost for cleaning, removing, and transporting the material to an approved disposal site. The waste on site shall be delivered and disposed to a designated facility in 6133 Edith Blvd NE, Albuquerque, NM 87107. The lump sum change order price includes all labor, materials, and equipment required to complete this work. The agreed upon price for all costs associated with this work is $2,895.59 in total.
