CHANGE ORDER NO. 8 February 20, 2014 Reason/Purpose for this Change Order: The office building floor substrate did not meet the floor manufacturers requirements for smoothness and the condition of the subfloor after removal of the existing flooring requires more than minor floor preparation initially proposed. This change includes the work and a material approved by the floor manufacturer to achieve a smooth even substrate prior to installation of the replacement flooring. This change order entails the additional cost for the work that shall be done for the interior flooring (2,135 ft2) of the Office Building. The City of National City and Contractor agree to the following change to the original Contract: The City and the Contractor agree that the Contractor shall prepare the floor according to requirement of the floor manufacturer which entails a combination of sanding and floating of an approved material to smooth the 2,135 ft2 interior flooring of the building. CHANGE ORDER NO.9 March 20, 2014 Reason/Purpose for this Change Order: Three monitoring wells 10" to 12" diameter with an approximate depth of 50' located along 22nd St. between Wilson St. and Hoover Avenue were installed in December of 2005 for the evaluation of water quality of a sewer trunk line. An inspection report on January 2008 and another on November 2013 confirmed that these wells are no longer needed. According to these reports, the DEH expects the monitoring wells to be destroyed as they are no longer required for sampling and data gathering activities and are an access point for possible contamination into the groundwater. Since the wells are no longer utilized, they are considered to be abandoned and must be properly removed and the pavement restored. The location of the sites are included in Exhibit "A". On January 16, 2008, I conducted an inspection of the wells. An inspection report, with a February 21, 2008 deadline to respond, was sent to Mr. Albert Griego. On January 25, 2008, I spoke to Mr. Griego on the telephone and he confirmed that the wells were no longer needed. I advised him to submit a permit application to destroy the wells and provided contact information for the driller who installed the wells. On March 11, 2008, I discussed the well destruction process with Mr. Griego on the telephone and extended the well destruction application submittal deadline until April 15, 2008. This deadline was not met. I attempted to contact Mr. Griego in March of 2010 and was informed he was no longer employed by the City of National City. On May 6, 2010, I sent a brief synopsis of the situation by email to Mr. Charles Nissley and requested a response. We have found no record of receiving a response or an application to destroy the wells. On November 14, 2013, I visited the site and confirmed that all three wells still exist. If the wells are no longer in use, they are considered abandoned and must be properly destroyed by permit through the MWP. According to San Diego County Code of Regulatory Ordinances, Chapter 4, Title 6, Division 7, Section 67.402, the terms "abandoned" or "abandonment' shall apply to a well which has not been used for a period of one year. Monitoring wells that remain on site, without continual maintenance, pose a threat to the groundwater because they present a route for the transport of surface contamination to the groundwater. They may also present an easy route for the illegal disposal of solids or liquids for which the property owner could be responsible. Within 60 days from the date of this letter, complete and submit the enclosed monitoring well destruction application with the permit fees to the attention of the Monitoring Well Permit Desk.
