CHANGE ORDER NO. 8 February 11, 2014 Reason/Purpose for this Change Order: There are 3 Mexican Fan Palms at the Northwest corner of 8th Street and F Avenue that are in conflict with the new improvements. The Engineer recommends that they be removed for the new planter area to be properly installed. The City of National City and Contractor agree to the following change to the original Contract: The City and the Contractor agree that the Contractor shall remove, stump and dispose of the 3 Mexican Fan Palms at the Northwest corner of 8th street and F Avenue. The agreed upon lump sum price for all costs associated with this work is $1,144.50, see Attachment "A" for cost breakdown. To fix make the grades ADA compliant.. The work proceeded by time and materials under the direction of the Engineer. The City of National City and Contractor agree to the following change to the original Contract: The City and the Contractor agree that the Contractor shall remove and dispose of the improvements to the curb, gutter and flatwork at Station 47+50 as directed by the Engineer to accomodate ADA compliance. CHANGE ORDER NO. 11 March 3, 2014 Reason/Purpose for this Change Order: The Engineer directed the contractor to remove the driveway at 333 8th Street because the grades were in conflict with the new sidewalk, curb and gutter improvements. The City of National City and Contractor agree to the following change to the original Contract: The City and the Contractor agree that the Contractor shall remove the existing driveway at 333 8th Street at a lump sum price of $277.20 and the installation of the new driveway will be charged to bid item no.10 "Construct PCC Driveways".
