Civil Construction CHANGE ORDER NO. 21 September 29, 2015 Reason/Purpose for this Change Order: Slurry the portion of paving between 14th/National City Blvd. to 12th on A Ave. for preventative maintenance. The City of National City and Contractor agree to the following change to the original Contract: The city has directed the contractor to provide a Type li slurry seal to approximately 33,000 sf of roadway on A Ave. between National City Blvd./14th to 12th St. This work will include all necessary "No Parking" posting, cleaning and preparation and coordination. All striping by the contractor will remain per plans, existing striping will be removed on a T&M basis and striping not shown on the plans will be replaced by city forces. The slurry shall be applied by October 9, 2015 and includes one mobilzation.
