Reason/Purpose for this Change Order:
The city has directed the contractor to remove and replace existing failed sidewalk and grade the adjacent
;slope as directed.
rThe City of National City and Contractor agree to the following change to the original Contract:
Remove approximately 100' of existing sidewalk, clear existing vegetation between the adjacent sidewalk and
=the drainage channel, remove existing silty soil, prepare a bench as needed, over -excavate and compact
;slope, import material as needed, grade sidewalk, repair the adjacent chainlink fence as needed and
reconstruct sidewalk. This change order includes all labor, equipment and materials needed to complete all
work described above.

price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor,

The city directed the contractor to install an additional pull box near the meter pedestal since one was not
existing as the plans stated.
The City of National City and Contractor agree to the following change to the original Contract:
The contractor shall install one #5 pull box to facilitate the installation of conduit to the meter pedestal near the
northwest corner of Paradise Valley Road and E. Plaza Blvd. not shown on the project plans.
