Reason/Purpose for this Change Order:
The city has determined asbestos materials were located in the El Toyon Restroom facility prior to demolition
and directed the contractor to properly remove and dispose the material.

The City of National City and Contractor agree to the following change to the original Contract:
The city determined the El Toyon Restroom may contain asbestos materials. After undergoing a testing
procedure the results came back positive. The contractor has been directed to obtain a contractor licensed
and registered to properly remove and dispose of the asbestos material identified in the El Toyon Restroom
roof mastic. This work shall include all labor, materials and equipment and related cost necessary to
complete the asbestos removal and disposal at the El Toyon Restroom facility. All work described above shall
be completed for the lump sum price stated below.

Reason/Purpose for this Change Order:
The City has directed the Contractor to delete a portion of electrical work related to camera installation for
security coordination with the City's IT Department.
The City of National City and Contractor agree to the following change to the original Contract:
The Contractor shall eliminate installation of 37 cameras and mounts, and associated system testing. This
change order deletes Bid Item #36 for $11,270.60 at Kimball Park; Bid Item #87 for $7,889.42 at El Toyon
Park; and Bid Item #145 for $1,690.59 and Bid Item #148 for $4,007.36 at Kimball Skate Park. This deduction
includes all associated labor and equipment related to installation, system testing and verification. All work
described above shall be completed for the lump sum price stated below.

The city has determined two existing light towers at Las Palmas Park should be removed immediately due to
public safety.
CHANGE ORDER NO. 9
May 5, 2016
The City of National City and Contractor agree to the following change to the original Contract:
The city determined that two existing light towers at Las Palmas Park had corrosion to the existing pole baes
that could fail without warning. The contractor has been directed to bring down two poles identified at Las
Palmas and completely remove them from the site. This work shall include all labor, materials and equipment
and related cost necessary to complete the removal and secure the existing electrical wires so the additional
remaining lights and electrical system is safe to the public. All work described above shall be completed for
the lump sum price stated below.

The city has determined temporary restroom are needed during the construction of the two El Toyon
Restrooms since there are no other facilities in place.
The City of National City and Contractor agree to the following change to the original Contract:
The contractor has been requested to supply temporary restrooms during the construction of the two El Toyon
Restrooms since there are no other facilities in place during construction. The contractor supplied two
standard and two ADA restrooms with sinks. The restrooms are to be serviced twice a week for a three
month period. All delivery and services are included for the lump sum price indicated below.

Reason/Purpose for this Change Order.
The contractor encountered an existing mainline during excavation of the Skate Park. The mainline was in
conflict with the construction of the new Skate Park and required relocation.
The City of National City and Contractor agree to the following change to the original Contract:
The contractor was required to relocate an existing 4" PVC mainline and irrigation wire outside the
construction limits of the Skate Park. All work was completed on a T&M basis.

Reason/Purpose for this Change Order.
The contractor encountered several locations of unmarked existing irrigation wire. The wire had been
previously spliced and buried below grade without proper identification or splicing methods and needed to be
repaired or replaced.
The City of National City and Contractor agree to the following change to the original Contract:
The contractor has been directed to trace, identify and repair as needed multiple locations of existing
irrigation wire. This included repairing existing faulting connections and marking all wire to three separate
controllers and installing splice boxes as needed. All work was completed on a T&M basis.
