CHANGE ORDER NO. 3 August 25, 2016 Reason/Purpose for this Change Order: To remove and replace additional sidewalk at the Granger Avenue Speed Table for ADA compliance, and for additional dump fees associated with petro mat fabric not identified on the plans The City of National City and Contractor agree to the following change to the original Contract: The contractor shall remove and replace an additional 486 SF of concrete sidewalk at the Granger Avenue Speed Table on both the east and west side of the improvements, as directed. The contractor also encountered petro mat fabric in the existing asphalt removed. This material is considered hard to handle by the recycling plant and requires a premium dump fee to be paid. All work and related costs shall be included in the lump sum amount, below. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Additionally, Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
