CHANGE ORDER NO. 6 September 23, 2016 Reason/Purpose for this Change Order: Core concrete footings to replace existing rusted foundations bolts during bridge refurbishment. The City of National City and Contractor agree to the following change to the original Contract: The contractor encountered rusted anchor bolts when removing the pedestrian bridge across Paradise Creek. In order to make sure the bridge was secured correctly the city directed the contractor to drill out the existing rusted foundation bolts and replace them with new bolts during the bridge removal, refurbishment and replacement. Additionally, Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
