CHANGE ORDER NO. 3 July 13, 2017 Reason/Purpose for this Change Order: Repair approximately 5400 SF of asphalt on Division St. just east of Highland Ave. damaged by an unknown diesel spill shortly after the roadway was paved by the contractor. The City of National City and Contractor agree to the following change to the original Contract: The contractor shall grind out, the area designated by the construction inspector in the field, (5417 SF +/-) existing asphalt to a depth of 3" and repave. The area shall be ground smooth, cleaned and swept as needed, tacked and repaved and includes all necessary resourced needed to complete the work including traffic control. A breakdown of this change order has been attached, however, the cost varies due to a 5% mark-up on the sub -contractor agreed to by the city in lieu of the contractor's proposed 10% mark-up. All work will be performed at the agreed price listed below. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Additionally, Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
