18th St. Pedestrian and Bicycle Enhancements Project CHANGE ORDER NO. 4
CIP No: 16-10 February 15, 2018
Contractor: Tri-Group Construction & Development, Inc.
Reason/Purpose for this Change Order:
Install electrical conduit for LED crossing at 18th and Lanoitan and remove and replace miscellaneous traffic
signal pull boxes. Also install 3" electrical conduit for SDG&E service.
The City of National City and Contractor agree to the following change to the original Contract:
The contractor has been directed to install the LED pedestrian crossing per the alternate bid item at 18th and
Lanoitan. The conduit work was not included with the alternate bid item and has been installed as extra work.
There were existing damaged traffic signal pull boxes at 18th and L and 18th and Newell. The contractor was
directed to remove and replace those pull boxes. The contractor was also directed to install a 3" conduit for
the SDG&E service. All work was performed on a T&M basis and this change order includes all labor,
materials and equipment necessary to complete the work as described.

By signing this Change Order the Contractor confirms that he/she is completely familiar with its terms and conditions and has fairly
negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and
labor necessary to provide a complete work as described above at that the price stated herein. Additionally, Contractor confirms that the
price is fair and complete and represents all costs associated with the work, including but not limited to all equipment. materials, labor,
supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions
of the original contract documents are incorporated herein. (2 originals: 1 for City Clerk, 1 for Contractor)
