Reason/Purpose for this Change Order.
The city has directed the contractor to perform additional potholing and explore an unknown utility conflict with
the new catch batch construction.
The City of National City and Contractor agree to the following change to the original Contract:
The contractor was directed to excavate additional locations to determine the depth and location of an
unknown water and gas line in conflict with the new catch basin construction. All worked was performed on a
T&M basis for the lump sum price listed below.

By signing this Change Order the Contractor confirms that he/she is completely familiar with its terms and conditions and has fairly
negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and
labor necessary to provide a complete work as described above at that the price stated herein. Additionally, Contractor confirms that the
price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor,
supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions
of the original contract documents are incorporated herein. (2 originals: 1 for City Clerk, 1 for Contractor)

Reason/Purpose for this Change Order:
The city has directed the contractor to construction eight ADA ramps at various locations along Harbison.
The City of National City and Contractor agree to the following change to the original Contract:
The contractor was directed to construct eight ADA ramps at various locations along Harbison at a unit cost of
$4000.00 each. The unit price includes removal of existing concrete and asphalt and all other work and
materials needed is construct ADA compliant ramps per SDRSD at Harbison and Division, Harbison and 2nd,
and Harbison and 4th. All worked will be performed for the lump sum price listed below.
