This Change Order modifies Contractor's and the City's obligations and rights under the
abovementioned contract. This change order is not effective until approved by the City.
Time adjustment:
Sixty (60) working days are added to contract time due to unforeseen delays for the delivery of traffic
signal materials and equipment.
Cost of Change Order:
The additional cost change for the setup and installation of temp signal:
$21,979.67

negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the
equipment, materials and labor necessary to provide a complete work as described above at that the price

Subject: Change Order No. 5
Change Order No. 5 provides for:
The installation and setup for the temporary signal at Highland Avenue and Walmart Driveway.
Supporting Information:
It was determined that the soil conditions were not suitable to support the existing standard when drilling
for the new foundation at the South West corner of Highland Avenue and Walmart Driveway. To be able to
safely excavate & install the new foundation, a new temporary signal will be implemented in the
intersection as determined by the engineer. See attachment for Change Order No.5 for the breakdown of
the labor, materials, and equipment included.
Time Adjustment:
It is recommended that sixty (60) working days be added to the contract time.
Cost Adjustment:
The total cost associated for this work is $21,979.67 excluding the implementation of traffic control which
will be included in a separate change order and will be measured through a T&M basis.
