PO Box 2322 La Mesa, CA 91943 May 9, 2019 This Change Order modifies Contractor's and the City's obligations and rights under the abovementioned contract. This change order is not effective until approved by the City. Time adjustment: One (1) working day are added to contract time due to the additional excavation for the park lights throughout the project site. Cost of Change Order: The additional cost change for the Excavation of the Park Lights: $1,736.86 Recommended by Project Manager: Date: 3 aS r r, By signing this Change Order the Contractor confirms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein. Supporting Information: The initial design for the park lights was a direct burial system into contaminated soil. The design was modified to a standard break away support system with concrete foundations in clean soil. This new system will allow the City to maintain all park lights without contacting the remaining contaminated soil. See Attachment A for the Time and Material Breakdown. Time Adjustment: It is recommended that 1 working day be added to the contract time. Cost Adjustment: The initial removal and disposal of contaminated soil was captured under original Bid Line Item #11 - Impacted Soil Handling (Export). The additional efforts went above and beyond this bid item and was determined to be tracked under time and materials for a total of $1,736.86.
