This Change Order modifies Contractor's and the City's obligations and rights under the
abovementioned contract. This change order is not effective until approved by the City.
Time adjustment:
Two (2) working days are added to contract time for the removal and disposal of additional asbestos
containing material.
Cost of Change Order:
The City and the Contractor agree that the contractor shall remove and dispose
approximately 2,646 SF of drywall and various asbestos tiles found throughout the
second floor of the 304 W. 19th Street site office. The agreed upon price for the
additional removal and disposal is a total lump sum price of $9,075.63.

By signing this Change Order the Con ctor confirms that he/she is completely familiar with and has fairly
negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the
equipment, materials and labor necessary to provide a complete work as described above at that the price
stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with
the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds,
insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the
original contract documents are incorporated herein. The signer for the Contractor confirms that he/she is
authorized to bind the Contractor to this Change Order.

Subject: Change Order No. 13
Change Order No. 13 provides for:
The removal and disposal of additional asbestos containing material at the 304 W. 19th Street site office.
Supporting Information:
The initial Hazardous Material Survey, dated March 8, 2019, identified 2,400 SF of asbestos materials to be
removed and disposed. As abatement began, additional asbestos containing materials were found.
Approximately 2,646 SF of drywall and various tiles throughout the second floor of the building were
discovered to contain asbestos. These additional asbestos containing materials were not listed in the
original Hazardous Material Survey.
The original Change Order #6 cost of $16,615.00 captured the removal and disposal of 2,400 SF of asbestos
materials that was originally identified in the initial Hazardous Material Survey. The additional removal and
disposal that was discovered when abatement began is approximately half of the original Change Order #6
cost to remove and dispose asbestos containing material. In order to benefit from having the contractor
mobilized and on -site with proper asbestos safety measures, it is fair and reasonable to accept the price of
$9,075.63 which captures all labor, equipment, materials, overhead and profit.
Time Adjustment:
It is recommended that 2 working days be added to the contract time.
Cost Adjustment:
The initial removal and disposal of asbestos containing material was approved under Change Order #6 for
this contract at a price of $16,615.00. The additional efforts for removal and disposal of the additional
asbestos containing materials are to be captured under an agreed lump sum price of $9,075.63. The pay
method and amount have been reviewed by the Construction Manager Inspector and are considered fair
and reasonable.
