This Change Order modifies the Contractor's and the City's obligations and rights under the
abovementioned contract. This change order is not effective until approved by the City.
Time adjustment:
Two (2) working days are added to contract time due to unforeseen delays for the delivery of and
installation of twenty (20) load switches.
Cost of Change Order:
The City and the Contractor agree that the Contractor shall install the additional
twenty (20) load switches in traffic signal cabinets that were not included in
the original set of plans as referenced in RFI #15. The agreed upon price for
all labor, materials, equipment, and appurtenances associated with this work
is $1,617.80.

Change Order No. 6 provides for:
The installation of additional (20) load switches in traffic signal cabinets at various locations
Supporting Information:
As per RFI #15, all traffic signal gears and supplies provided and listed in Construction Note #1. During the
installation of controllers in the cabinets, it was observed that load switches were missing because it was not
listed in the construction note. The designer reviewed and provided a direction to quantify the number of
load switches and proceed with the installation. The Contractor identified that twenty (20) additional load
switches will need to be installed.
Time Adjustment:
It is recommended that two (2) working days be added to the contract time.
Cost Adjustment:
The total cost for installing the twenty (20) additional Toad switches is $1,617.80
