CHANGE ORDER NO. 1 November 12, 2019 Reason/Purpose for this Change Order: The city has directed the contractor to excavate and construct a 12" thick aggregate base access road in lieu of the dirt access road shown on the project plans. The City of National City and Contractor agree to the following change to the original Contract. The contractor has been directed to excavate and remove a 12' wide by 9" section of subgrade and place a 12' wide by 12" section of aggregate base with tapered edges for the length of the access road shown on the plans. All work including all labor, equipment and export of subgrade material will be performed for the lump sum price listed below. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Additionally, Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein. This proposal shall be null and void if written acceptance and notice to proceed are not received within 30 days. 6. This proposal does not include engineering, inspection, permits or fees unless noted above. 7.
