Reason/Purpose for this Change Order:
The city has directed the contractor to modify traffic signal mast arms and install one additional pole to avoid
conflict with the SDG&E overhead power lines per the revised plans.
The City of National City and Contractor agree to the following change to the original Contract:
The contractor is directed to make necessary modification per the revised traffic signal plans by Kimley-Horn
installing modified luminaire arms on poles A and H at Windsor, modifying pole E from a Type 15TS to a 1A
and installing a new pole G at 6th and Euclid. This change order includes all necessary work to accommodate
the modification and installation the these poles including all additional labor, equipment and materials
necessary to complete all work excluding the modification to the ADA ramp at 6th and Euclid. The contractor
has requested a deferment of additional days until a lead time of materials can be determined. All work will be
performed at the total lump sum price listed below including the contractor's 5% mark-up.

By signing this Change Order the Contractor confirms that he/she is completely familiar with its terms and conditions and has fairly
negotiated its price, terms and conditions. If this Change Order is approved. Contractor will provide all of the equipment, materials and
labor necessary to provide a complete work as described above at that the price stated herein. Additionally, Contractor confirms that the
price is fair and complete and represents all costs associated with the work, including but not limited 10 all equipment, materials. labor.
supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions
of the original contract documents are Incorporated herein. (2 originals: 1 for City Clerk, 1 for Contractor)

Scope of Work Under this Change:
Euclid Avenue @ Windsor Heights:
-Pole A — Remove & Install 8' Mast Arm (return Original 15' LMA to City) (note 26): $3,792.50
-Pole H — Additional Mobilization to install Pole H: $1,678.50
Euclid Avenue @ 6th & 7th:
-Pole E — Adjust existing foundation from 15TS to 1-A;
Furnish & Install 1-A Pole; extend conduit & install 1 EA #5 pullbox (PCI to restore the ramp): $5,724.92
-Pole G — Augur & Pour 15TS foundation; install additional conduit & wire: $4,187.10
Total COR Amount = $15583.02

SEI proposes to perform the above -described extra work on the following basis:
Lump Sum Price of Fifteen Thousand Five Hundred Eighty Three Dollars and Two Cents

performance of this extra work will require a time extension of TBD Calendar Days.

Please Note: The price quoted is based on work being performed under normal conditions including reasonable access to the work. The proposed price does not include extra costs
for performance of work out of sequence, schedule delays, acceleration, extended overhead, or any other impacts caused by the cumulative effect of changes on original contract
work. Select Electric Inc. hereby reserves all rights to re-evaluate its costs and seek equitable adjustment should it become evident that costs have been impacted by any of the
above causes. Should it be impractical to determine any time impacts at the time of submittal of this Proposal, Select Electric Inc. reserves its rights to seek appropriate time
extensions should it become evident at a later date that this extra work contributed to project schedule delays.
