This change order also provides for the installation of filter fabric, boulders and decomposed granite per the revised 2"d and Sweetwater plan. Supporting Information: This change order includes additional preparatory work at the SE corner of 2"d and Sweetwater to raise the grade to meet revised sidewalk, curb & gutter, and bike lane grades. These revisions were made to reduce the elevation differences between the original plan curb and gutter and the existing asphalt section. Making these revisions, also revised the flatwork improvements which added a decomposed granite area with multiple sized boulders throughout this area to fill in the open space between the bike path and the curb & gutter. Attachment A — 2"d & Sweetwater Revised Plan Time Adjustment: Twenty -Five (25) working days are added to the contract time to capture all work described above. Cost Adjustment: All work was captured under a time and material basis. The agreed upon price for all associated work totals to $19,324.90. This change order is not effective until approved by the City. Time adjustment: Twenty -Five (25) working days are added to contract time for additional import and grading of Class II base, installation of filter fabric, boulders and DG at the SE corner of 2"a Street and Sweetwater Rd. Cost of Change Order: The City and the Contractor agree that the Contractor shall proceed under a Time and Material basis for the additional import, grading and compaction of Class II base to raise the grade within the DG footprint in the SE corner of 2"a and Sweetwater. The City and the Contractor agree that the Contractor shall proceed under a Time and Material basis to install filter fabric, boulders, and decomposed granite per attached revised plans. DEPT. 1010 APR ?0 A 10: 41 CITY OF NATIONAL CITY Change Order No. 12 provides for: The additional mobilization for traffic signal installation at 16th Street and National City Blvd. Supporting Information: During installation of the traffic signal poles at 16`h Street and National City Blvd, the traffic signal pole at the North East Corner did not fit the foundation/bolt pattern. The foundation was built per plan for a 24 pole. However, per manufacturers requirement a 24 pole cannot be installed with a 40-foot mast arm as called out on Sheet 11 of the contract plans. At this time, the Design Engineer was contacted to review options of traffic signal poles that will satisfy site conditions. After reviewing site conditions, the Design Engineer confirmed that a 24 pole with a 35-foot mast arm will satisfy manufacturer requirements and provide safe traffic conditions at this intersection. The contractor remobilized and installed the approved traffic signal pole and equipment. The foundaton was built per plans fora 24 pole. The pole manufacturer stated that you cannot Install a 40' mast arm on a 24 pole which is why they received a 26 pole. If you refer to sheet 11 of the plans the table calls out 24 pole with 40' mart arm. I attached of the different poles and mast arm lengths. They are going to stand the old pole again until this gets figured out. One option would be Installing a 24 pole with a shorter mast arm if possible but there might be a long lead time.
