CHANGE ORDER MEMORANDUM May 20, 2020 To: Roberto Yano, P.E. & Jose Lopez, P.E. Director of Engineering & Public Works/ Deputy City Engineer Engineering and Public Works Department City of National City From: Alex Preciado Project Manager Project Professionals Corp Project: Paradise Creek Park Site Remediation & Improvements CIP No.: 18-07 Subject: Change Order No. 28 Change Order No. 28 provides for: The re -excavation for 6 streetlight foundations to lower the pole bases 4-inches below finish surface. Supporting Information: The original CIP No. 18-07 project plans identified the footing detail for the streetlights that were installed throughout Harding Ave and 21' Street. The detail showed the original footing to sit approximately 4-inches above the finish surface of sidewalk. With this detail an additional 4-inch concrete cap would have to be poured around the anchor bolts to serve as a surface for the base of the pole to sit on. However, to delete additional concrete caps within the sidewalk limits, the onsite inspector confirmed that the bases can be flush with the sidewalk and no concrete cap would need to be poured around the anchor bolts. At this time the Contractor excavated an additional 4-inches for all 6 streetlight foundations. This lowered the pole base and allowed for the pole base to be flush with the new sidewalk improvements. Having the base of the pole flush with the sidewalk will allow for easy City maintenance and increase sidewalk accessibility around each streetlight. Time Adjustment: It is recommended that 0 working days be added to the contract time to excavate an additional 4-inches to lower the base of the poles to allow them to be flush with the sidewalk finish surface. Cost Adjustment: All work associated with the additional excavation was captured under a Time and Material basis. The total amount for labor, material, and equipment associated with this work is $1,086.75. This change order is not effective until approved by the City. Time adlustment: Zero (0) working days are added to contract time for the re -excavation of 6 streetlight foundations to Tower the pole bases 4-Inches. Cost of Change Order: The City and the Contractor agree that the Contractor shall proceed under a Time and Material basis to re -excavate 6 streetlight foundations to lower the pole bases 4-inches below finish surface to allow for the base of the pole to be flush with the finish surface of the sidewalk. The total amount for all labor, material and equipment associated with this work is $1,086.75. $1,086.75 Recommended by Project Manager: Date: 5/z4/202.c> By signing this Change Order the Contra or confirms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein. This required re -excavating the all the holes to make sure the bases complied with detail and rebar cage could fit. Relocate pole base on 21st St. and Harding 10' from original survey per inspector.
