Director of Engineering & Public Works, Deputy City Engineer Engineering and Public Works Department City of National City From: Alex Preciado Project Manager Project Professionals Corp Project: Paradise Creek Park Expansion CIP No.: 18-16 Subject: Change Order No. 13 Change Order No. 13 provides for: The additional work to encase 3 electrical conduits in concrete above an existing storm drain at Paradise Creek Park. Supporting Information: The original plans called out for 3 1-inch conduit and 1 2-inch conduit to be installed at Paradise Creek Park to support the new security camera system and irrigation controls for the park. However, when trenching along 19`h street, the Contractor encountered an unmarked storm drain unit at approximately 17" deep. At this time, City staff was requested to the site and confirmed this is an active storm drain unit. The contractor submitted RFI #14, requesting if the electrical conduits can be installed above the storm drain unit and encased with concrete to protect the infrastructure. The Design Engineer reviewed site conditions and current electrical code requirements and confirmed the conduit duct may be a minimum of 12-inches below grade and encased with a minimum of 2-inches of 3000PSI concrete. This will minimize City maintenance and reduce risk of damage to the infrastructure. Time Adjustment: It is recommended that 0 working days be added to the contract time for the work described above. Cost Adjustment: Having the conduits encased with concrete was the most cost-effective option to continue production on site. The alternative option for this, would have been to trench under the existing storm drain unit, approximately 36"deep, which would have needed additional time and bigger equipment. This would have created future accessibility issues for maintenance since the duct would be beneath an existing storm drain unit. All parties were in agreement to continue the work described above under a Time and Material basis. The total agreed upon price for all labor, materials, equipment and all other associated work is.447865.89.' This change order is not effective until approved by the City. Time adjustment: Zero (0) working days are added to contract time for the additional work to encase 3 electrical conduits in concrete above an existing storm drain to meet electrical code requirements. Cost of Change Order: The City and the Contractor agree that the contractor shall proceed under a time and material basis to encase 3 electrical conduits in concrete above an existing unmarked storm drain unit per attached RFI ff14 response. All associated work, labor, equipment and materials required to complete this work totals to an agreed lump sum price of $1,928.10. $1,928.10 Recommended by Project Manager: Date: Sizoi.w By signing this Change Order the Contra or confirms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
