Deputy City Engineer City of National City From: Alex Preciado Project Manager Project Professionals Corp Project: Paradise Creek Park Expansion CIP No.: 18-16 Subject: Change Order No. 14 Change Order No. 14 provides for: The design and structural calculations of 2 play structures at Paradise Creek Park. Supporting Information: Original contract documents and plans identified two play structures to be installed within the playground area at Paradise Creek Park. Contract plans showed an allowable depth of 24-inches for a footing to support the play structures. However, during construction it was determined that the manufacturer could not meet the allowable depth for a footing to support their equipment. The original depth for the footing for both structures is between 48-inches to 60-inches. At this time, it was determined to consult with Oriel Structural and Bridge Engineers to assist with the design of spread footings for these structures. Drawings and calculations were provided to the manufacturer and approved. The play structures are still fully warranted upon acceptance of the project. Time Adjustment: It is recommended that 0 working days be added to the contract time for the re -design of the 2 play structure footings. Cost Adjustment: The total price for the design and structural calculations $3,000.00. This change order is not effective until approved by the City. Time adjustment: Zero (0) working days are added to contract time for the design of 2 structural footings for 2 play structures at Paradise Creek Park. Cost of Change Order: The City and the Contractor agree that the contractor shall coordinate with Oriel Structural and Bridge Engineers to design 2 structural footings for the 2 Dynamo play structures at Paradise Creek Park. The total price for the design and structural calculations for 2 play structures is $3,150.00. Recommended by Project Manager: $3,150.00 Date: C�//r�,ra By signing this Change Order the Contra confirms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein. The signer for the Contractor confirms that he/she is authorized to bind the Contractor to this Change Order.
