Deputy City Engineer Engineering and Public Works Department City of National City From: Alex Preciado Project Manager Project Professionals Corp Project: Camacho Gym Roof Improvements CIP No.: 19-51 Subject: Change Order No. 1 Change Order No. 1 provides for: The removal and disposal of 4 existing HVAC ducts at Camacho Gym. Supporting Information: The original contract identified approximately 50 square feet of asbestos containing duct seal to be removed and disposed. As abatement began, the Contractor notified the City that the existing ducts were completely corroded. The Contractor would peel back the duct seal and while peeling back the duct seal the existing metal would also be removed. Because of the existing conditions of the ducts, proper abatement could not be accomplished. At this time, the City agreed to have the 4 ducts removed to properly remove all asbestos containing material and allow for the TPO membrane to be properly installed. Removal of the ducts would minimize City maintenance and also allow for a new full HVAC system to be installed at a later date by others. Time Adjustment: There is no added time to the contract for the work described above. Cost Adjustment: The initial removal and disposal of asbestos containing material at bid time was for a contract price of $4,600. The additional efforts for removal and disposal of the 4 HVAC ducts are to be captured under an agreed lump sum price of $2,660.00. This change order is not effective until approved by the City. Time adjustment: Zero (0) working days are added to contract time to remove 4 existing HVAC ducts. Cost of Change Order: The City and the Contractor agree that the Contractor shall remove and dispose of 4 existing HVAC ducts to allow for proper removal and disposal of asbestos containing duct seal. The agreed upon price for this work is $2,660.00 $2,660.00 i1We Date: 54, /Pp2G Recommended by Project Manager: By signing this Change Order the Contractor confirms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
