Supportinq Information: The Contractor submitted original USA Shade Structure shop drawings for all 4 shade structures at Paradise Creek Park. The Landscape Architect took no exception to the shop drawings submitted. The shade structure footing detail showed all 14 shade structure columns to be anchored and surface mounted to the footing with original column lengths hat varied between 8-ft and 11-ft. The Contractor began to excavate the footings per the original shop drawings, however during this operation, the Contractor discovered that site conditions did not reflect the allowable depth for the original footing design. The contract plans show an allowable depth of 36-inches for the shade structure footings, resilient surfacing, and base. The USA Shade Structure footing details a 30-inch deep footing, but after the Contractor began to excavate for this footing, site conditions only allow for an 18-inch deep footing plus 12-inches for base and resilient surfacing. At this time, the Contractor submitted an RFI requesting for direction and if a re -design of footings will be needed. After review of the site conditions and the original USA Shade Structure footings, it was determined that spread footings were going to be needed to meet site conditions. USA Shade Structures submitted re -designed spread footings for review. The Landscape Architect reviewed the revised detailed footings and took no exception. This re -design modified the installation of the columns within the footing. The original footing detail identified the columns to be surface mounted above the footing. However, the re- design footing detail identifies the columns to be directly embedded into the footing. Because of this modification, the original column lengths of 8-ft and 11-ft would not work. Because of this, approximately an additional 10-inches had to be added to the column lengths to accommodate for the direct embedment of the columns. Time Adjustment: Zero working days are added to the contract time for this change order. Cost Adjustment: The additional costs for the columns are to accommodate the additional length added to the original columns. The total agreed upon price for the additional materials costs and all other associated work is $3,018.79. ect Manager lita /imp Date CAI NIA -e. . . This change order is not effective until approved by the City. Time adjustment: Zero (0) working days are added to contract time for the additional lengths added to the 14 shade structure columns. Cost of Change Order: The City and the Contractor agree that the Contractor shall procure, deliver and install 14 shade structure columns per attached revised USA Shade Structures Drawing #71753-1.2, 71753-2.2, 71753-3.2. The City and the Contractor agree that this revised detail added various lengths to the original columns due to the direct embedment installation of all 14 columns. The Oty and the Contractor agree that all additional material costs, labor, equipment, overhead, and all associated costs to complete this work totals to an agreed lump sum price of $3,018.79. Recommended by Project Manager: $3,018.79 By signing this Change Order the Cont ctor confirms that he/she is completely familiar with and has fairly negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the equipment, materials and labor necessary to provide a complete work as described above at that the price stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds, insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the original contract documents are incorporated herein.
