Change Order No. 1 provides for:
Providing and installing additional asphalt to expand the ADA parking spaces in order to satisfy ADA requirements
and reduce grade differences to match existing pavement.
Per Contract Section 3-2.2.2 — Increase of More Than 25 Percent, Line item 5 — "Asphalt Paving" tonnage increased
more than 25 percent and a negotiated/agreed upon $275 per ton would be paid for the additional asphalt. After
line item 5 is fully paid out this change order covers the cost difference for the additional tonnage.
Supporting Information:
While setting curb elevations, the Contractor identified a need to increase parking space dimensions for ADA code
compliance and additional asphalt was needed to widen the west side parking space from nine (9) to twelve (12)
feet for ADA van accessible and to lengthen all ADA spaces from seventeen (17) to eighteen (18) feet. Additional
asphalt was also to be used to reduce the grade differences to match the existing pavement.
Time Adjustment:
No additional time required as the additional asphalt can be placed within the same working day.

Cost for this change order was calculated at an agreed upon unit price of $275/TN for a total of 28.1 tons. The
contracted 21 tons of asphalt was paved at the contracted unit price of $300%TN. The additional 28.1 tons were
paved at the negotiated price of $ 275, TN for a total cost of $7,727.50.
The pay method and amount have been reviewed by the Construction Manager and are considered fair and
reasonable. The supporting documentation is on file with the project records.

This Change Order modifies Contractor's and the City's obligations and rights under the abovemeritioned
contract. This change order is not effective until approved by the City.
Time adjustment:
Zero (0) Working Days are being added to this contract.
Cost of Chan a Order:
The City and the Contractor agree it iz5 the Contractor shall proceed with providing and installing
additional asphalt to expand the ADA parking spaces in order to satisfy ADA requirements and reduce
grade differences to match existing pavement. Per Contract Section 3-2.2, — Increase of More Than 25
Percent, Line item 5 — „Asphalt Paving" tonnage increased more than 25 percent and an agreed upon
$275 per ton would be paid for additional tonnage. This change order covers the cost for the additional
tonnage at the renegotiated price of $275 per ton. The total amount for all labor, material, equipment
and all other associated work is 7,727.50.

By signing this Change Order the Contractor confirms that he/she is completely familiar with and has fairly
negotiated its price, terms and conditions. If this Change Order is approved, Contractor will provide all of the
equipment, materials and labor necessary to provide a complete work as described above at that the price
stated herein. Contractor confirms that the price is fair and complete and represents all costs associated with
the work, including but not limited to all equipment, materials, labor, supervision, overhead, fees, bonds,
insurance, profit, etc. and that there will be no further compensation. All of the terms and conditions of the
original contract documents are incorporated herein. The signer for the Contractor confirms that he/she is
authorized to bind the Contractor to this Change Order.
